Health Information Exchange. If Agency is qualified to enroll in a health information exchange, the City encourages Agency to do so in order to facilitate the secure exchange of data between Agency’s electronic health record system (EHR) and the City’s Epic EHR.
Health Information Exchange. Participating Provider agrees to participate in any Highmark, Highmark sponsored, and/or Highmark-designated community sponsored health information exchange program, which supports and/or facilitates the availability and exchange of claims- based information and clinical information for the treatment and ongoing management of Highmark members and/or other patients of Participating Provider. Any requirements relating to participation in such health information exchange program shall be provided through Highmark communications and/or Administrative Requirements in advance of such participation requirement and shall be binding on Participating Provider and Highmark.

Health Information Exchange. (HIE), when used as a verb, means the electronic movement of health-related information among organizations according to nationally recognized standards with the goal of facilitating access to and retrieval of clinical data to provide safe, timely, efficient, effective, equitable, patient-centered care. An HIE, when used as a noun, refers to an organization that collects health information electronically, manages it, and makes it available across the health care system.

Provider must provide to Subcontractor or Health Plan and/or the Department all information the Department requires under the applicable State Contract related to the performance of Provider’s responsibilities, including encounter data, as well as non- medical information for the purposes of research and evaluation, and any information the Department requires to comply with all applicable federal and State laws and regulations. As applicable, Provider shall submit to Subcontractor or Health Plan all risk adjustment data as defined in 42 CFR 422.310(a). Provider shall send to Subcontractor or Health Plan all risk adjustment data and other Medicare Advantage program-related information as may be requested by Health Plan, within the timeframes specified and in a form that meets Medicare Advantage program requirements. By submitting data to Subcontractor or Health Plan, Provider represents, and upon Subcontractor’s request shall certify in writing, that the data is accurate, complete, and truthful, based on Provider’s best knowledge, information and belief.

To deliver safe, effective, high quality and affordable care in the 21st Century, strategic adoption of an interoperable health information infrastructure is needed to transform health care from today's largely paper-based, disconnected system to an electronic, interconnected health care system. Accordingly, as one of its principal health care reform initiatives, New York has engaged in the development and implementation of a health information infrastructure. Broad adoption and use of health IT is vital to the Governor’s vision for health care in several ways. It plays a significant role in our progress to ensure that clinical information is in the hands of clinicians and New Yorkers so that it guides medical decisions and supports the delivery of coordinated, preventive, patient-centered and high quality care. Health IT can gather more precise and timely information about what works in the real world to refine health care policies, monitor health status and safety and guide physician and patient treatment choices. Health IT can replace expensive, stand-alone health surveillance systems with an integrated infrastructure to allow for seamless health information exchange for many public health purposes. Health IT can provide timely information about choices, prices, quality, and outcomes – information essential to a patient-centered health care system. However, health IT alone will not result in the expected quality and population health improvement and efficiency goals. Key alignment of health IT with public health and clinical practice models, new quality and outcomes-based reimbursement models, prevention and wellness initiatives as well as services to support clinicians in learning how to consistently use information to realize the value are essential to improve quality, affordability and outcomes for all New Yorkers. Coordination with Medicaid, other state health programs and private payers is essential to achieve this broader objective.
