Monthly Membership Fee. Service Provider shall be charged a monthly membership fee of twenty-nine dollars and ninety- nine cents ($29.99) on a monthly basis if Service Provider chooses to join the Platform once Service Provider has cleared each step of the onboarding process as set forth in Exhibit B. Fee Percentage The percentage determining the fee shall be set based on the number of appointments or sessions (collectively, the “Sessions”, and each a “Session”) with the Service Provider performs in any Week on the Platform as set forth below: • Five (5) or fewer Sessions in any week — eighty-two percent (82%) of the Client Payments (as defined below) from such Sessions performed during such week;

Monthly Membership Fee. Your account has a Monthly Membership Fee that will be billed to your account each month as a charge, whether or not you use your account, and applied against your available credit like other charges. The Monthly Membership Fee amount is listed in the Fee Schedule. Your payment of the Monthly Membership Fee does not affect our right to close your Account or limit your right to make transactions on your Account.
Monthly Membership Fee. The amount of your monthly membership fee (“Membership Fee”) is set by your Circle Leader and specified on their registration page. The Membership Fee is paid initially, and then every 30 days thereafter the intial payment. Payments shall be made through the WomanSpeak online registration form via credit or debit card. The monthly membership fee will be charged by WomanSpeak to the credit or debit card that you register with us. If you wish to change the credit or debit card on file, you must email WomanSpeak at xxxxxxx@xxxxxxxxxx.xxx at least 15 days prior to the next payment.
Monthly Membership Fee. Membership fees start on the date of enrollment and are automatically billed monthly on the same date each month. Any fees or charges that are not included in the Membership Fee (i.e. fees for noncovered services, such as labs, medications, procedures, etc.) shall be due at the time of service. I understand that I may not prepay the fees associated with this agreement.
Monthly Membership Fee. 3.2.1 During each month of membership in the SA Membership Program, SA Member agrees to pay XO a monthly membership fee in the amount of $250.00 (the “Monthly Membership Fee”). XO shall deduct an amount equal to the Monthly Membership Fee on the first day of each calendar month throughout the Membership Term from Member’s Membership Account. For any partial month of membership upon enrollment in the SA Membership Program, the Monthly Membership Fee shall be pro-rated and deducted from the Initial Membership Account Deposit.
Monthly Membership Fee. The Member shall also be required to pay a monthly membership fee for maintaining the Member’s Supply in the selected membership program the Member requested at the time of the Member’s Application. This monthly membership fee may change, depending on the cost to maintain the Member’s Supply and notice of such a change shall be delivered to the members at least thirty (30) days before the change in pricing is implemented. The monthly membership fee shall be due by the first day of each month. The monthly membership fee shall automatically renew monthly. Monthly Membership Fee. In exchange for the Services described above, Program Member agrees to pay Practice a Monthly Membership Fee (“Monthly Membership Fee”). The Monthly Membership Fee is due in advance for each month you participate in the Practice, and is payable on the same day of the month that the initial payment was made (the “Payment Date”).
