Required Notice of All Claims. The Company and Executive agree that the aggrieved party must give written notice of any claim to the other party. Written notice to the Company, or its officers, employees or agents shall be sent to the Company’s Chief Executive Officer. Executive will be given notice at the last address recorded in his/her personnel file or such other address as Executive may provide to the Company from time to time following the date of this Agreement by a writing specifying that it is the address for notice under this Agreement. The written notice shall identify and describe the nature of all claims asserted and detail the facts upon which such claims are based. The notice shall be sent to the other party by certified or registered mail, return receipt requested.

The Demand for Arbitration shall be signed by the initiating Party, identify and describe the nature of all claims asserted, the facts upon which such claims are based, and the relief or remedy sought, including the monetary value of the amount in controversy. Any request for injunctive relief, declaratory relief, or attorneys’ fees shall not count toward the calculation of the amount in controversy unless such injunctive or declaratory relief seeks the payment of money.
