Service Request. An Enrollee’s oral or written request of the Contractor to authorize and pay for a benefit or service. This request for services shall include Covered Services as referenced in Appendix A and defined in Appendix B. Service Requests may also be referred to as: requests for Covered Services, requests for coverage decisions or requests for organization determinations.
Service Request. Contractor shall be assigned each Project via Service Request by the County which shall referred to as a “Service Request.” A Service Request for each Project shall be developed by Contractor in conjunction with the County Project Manager. The County Project Manager shall manage all the Contractor’s work including monitoring the Service Request schedule, quality of deliverables, review invoice amounts, adherence to set budget, and internal review of submittal packets. Contractor shall follow all the requirements in this Scope of Work. The Service Request shall include a detailed Scope of Work, describing the tasks to be performed with a specific list of deliverables for each task, schedule of work and cost to complete the work. The Service Request shall include a cover sheet provided by the County Project Management staff with the appropriate signature blocks and contract information. Once both Parties agree, and all parties have signed Service Request, County Management staff shall provide the Contractor with a Notice to Proceed (“NTP”) to begin work. No individual Project or Service Request under this Contract shall exceed $60,000. Projects or Service Request shall not be intentionally split or separated into smaller Projects or Service Request for the purpose of evading this provision.

Agency will promptly take steps to remove any Objectionable Display of Client Advertising upon becoming aware of the same. However, no liability shall attach to Agency in respect of any losses suffered by Client or by any third party by reason of: (a) the reliance of Client on any estimates or targets; or (b) any Objectionable Display of Client Advertising. Client understands and acknowledges that a majority of search engines and third party ad-servers and related technology providers, including, without limitation, Google and Yahoo as well as online or other media research providers or publishers and social media platforms (including Facebook) (collectively “Vendors”), through such Vendor’s terms of service and other policies, restrict users from engaging in certain activities and/or displaying certain content on or through the use of the Vendor’s services and/or require certain specific privacy disclosures (the “Vendor Requirements”). Notwithstanding anything in the Agreement, Client agrees to abide by such Vendor Requirements and agrees that Agency is not responsible for any activities undertaken by Client, or that are approved by Client that violate such Vendor Requirements. Further, Client agrees that it will ensure that its web site(s) will feature an easy-to-understand privacy policy and, if applicable, any other privacy disclosures that are necessary to comply with all applicable laws and regulations (including any applicable industry self-regulations generally and any Vendor Requirements.
