• The University of Connecticut Health Center and/or one or more of its component entities (including, but not limited to, the UConn School of Medicine, UConn School of Dental Medicine, UConn Xxxx Xxxxxxx Hospital, UConn Medical Group, UConn Health Partners, University Physicians, University Dentists and UConn Health Pharmacy Services, Inc.) • EOHHS will supply the Contractor on a daily basis for RIte Care members and on a monthly basis for Rhody Health Partners and ACA Adult Expansion members, with a list of members newly enrolled into the Health Plan, as discussed in Section 2.05.04 below.

• EOHHS will provide the Contractor with a monthly full roster of all members enrolled into Rhody Health Partners.

• INSURANCE AUTHORIZATION Assignment of Insurance Benefits: I hereby authorize payment of medical benefits directly to Aulike Health Partners LLC.

• If a parent/legal guardian is bringing a minor in for services who is under the age of 14, the consent of both parents or legal guardians is required by Aulike Health Partners LLC.

• Treatment of Minors: Permission to Treat a Minor (Under age 18): In the event of an emergency and I cannot be contacted, I give my permission to Aulike Health Partners LLC to treat my child in their office or appropriate treatment facility as required by the events of that emergency situation.
