Interest Form definition

Interest Form means an application for consideration submitted by a person expressing interest in serving on the Certified Burn Manager Advisory Committee or subcommittee.¶
Interest Form means the Data Exchange Interest Form above that Government Agency completed before viewing these Terms and Conditions.
