Each employee shall have his/her choice of one of the options listed below: PLAN A – MESSA Choices $500/$1000 with 100% co-insurance PLAN C – MESSA Choices $2000/$4000 with 100% co-insurance PLAN D – MESSA ABC 1 $1400/$2800 with 100% co-insurance (eligible for health savings account (HSA) funded by the employee) Employees enrolled in Plans A, C, or D also receive ancillary benefits in Plan B.
The Employer shall pay the following subsidies towards either Plan A or Plan B (prorated if less than 1.0 FTE) in 2017: • Single Subscriber - $447.29/month • 2-Person Subscriber - $966.42/month • Family Subscriber - $1,217.94/month Effective January 2018 and January 2019, the Employer and Employee will split renewal increases equally up to the maximum amounts permitted in Section 3 of the Publicly Funded Health Insurance Contribution Act.

Employees shall be provided with a dental plan covering eighty percent (80%) of the cost of basic services (Plan A) to an annual maximum of $1,500 per person per year, sixty percent (60%) of the cost of major services (Plan B) to an annual maximum of $1,000 per person per year, and sixty percent (60%) of the costs of the orthodontic services, (Plan C).
For Plan B, co-premium amounts will be as follows: Effective July 1, 2012, all members in the single coverage plan will pay one and one-quarter (1.25%) per cent of their straight time hourly rate of pay, excluding overtime, towards the cost of health insurance.
