CONCLUSION OF THE INSURANCE. 1. Bulstrad Travel Package insurance shall be concluded in accordance with the relevant provisions of the Insurance Code, according to a procedure and in a manner determined by the Insurer, for which a policy shall be issued. 2. The insurance contract is a set of documents including a policy, general conditions, clauses, endorsements, medical form, questionnaire (when required by the Insurer) and any other documents agreed upon by the parties and bearing the policy number. 3. Upon conclusion of the insurance, the Insured is obliged to report all material circumstances related to the risk assessment of which the Insurer has asked questions in writing. The Insured is liable for the truthfulness, accuracy and completeness of the information provided by him/her. 4. The insurance contract may be concluded in the following forms: 4.1. individual insurance; 4.2. group insurance. 5. Upon conclusion of a group insurance, all persons included in the policy shall be subject to the same cover and conditions, with the individual sum insured specified separately for each individual person. 6. The insurance policy shall mandatorily include CLAUSE 1 MEDICAL EXPENSES, and the insurance cover may be extended with one or more of the following additional clauses: ⚫ CLAUSE 2 LEGAL ASSISTANCE ⚫ CLAUSE 3 GENERAL THIRD PARTY LIABILITY ⚫ CLAUSE 4 CANCELLATION, INTERRUPTION OR EXTENSION OF THE VOYAGE ⚫ CLAUSE 5 DELAY, DAMAGE, LOSS OR THEFT OF LUGGAGE AND DOCUMENTS ⚫ CLAUSE 6 FAMILY ⚫ CLAUSE 7 SEARCH AND RESCUE EXPENSES ⚫ CLAUSE 8 ADDITIONAL ASSISTANCE SERVICES ⚫ CLAUSE 9 ACCIDENT ABROAD 7. Any special conditions amending the conditions of the insurance policy shall be specified in an endorsement to it. 8. No individual insurance may be concluded which covers the risk of “Death due to an Accident” for Minors.
