Common use of Admission/Physician Clause in Contracts

Admission/Physician. You must have a physician order to be admitted to the Facility. You appoint , a licensed New York physician, as your personal physician to oversee your medical care throughout your stay at the Facility. You agree to a physician visit at least once every thirty (30) days for the first ninety (90) days after admission and at least once every sixty (60) days thereafter and more often when medically indicated.
