Adopted Children. A legally adopted child or a child for whom a Subscriber is a court appointed legal guardian, and who meets the definition of a Covered Dependent, will be treated as a dependent from the date of adoption or upon the date the child was placed for adoption with the Subscriber. “
Adopted Children. For adopted children who are not newborns, the Effective Date of any coverage provided by AvMed will be the moment of placement in the Subscriber’s residence; however coverage shall not be effective if the child is not ultimately placed in the Subscriber's residence, in compliance with Chapter 63, Florida Statutes.
Adopted Children. For members who have been adopted, the Member’s region of residence shall be determined by the adoptive parent’s official residence. Adult Guardianship. For members who are in adult guardianship status, the county of residence shall be where the Member is living. Brief absences, such as for respite care or hospitalization, not to exceed one month, do not change the county of residence. The DCBS shall notify the Department when a Member’s case is transferred to another area. The Department will include notice of the transfer in the HIPAA 834. For former xxxxxx children under the age of 26 covered by the Expansion of Medicaid by the ACA, the county of residence shall be where the Member is living.
Adopted Children. Adopted children are considered eligible from the date of assumption of legal responsibility. The adopted child is not automatically added to your policy. For Coverage to begin, an application form to add the child as a Dependent and a payment of any applicable premium must be received by us within thirty-one (31) days from the date of legal assumption. The premium shall include the first thirty-one (31) days of coverage. If the application and premium are not received by us within thirty-one
Adopted Children. For all children covered as Adopted children, if the final decree of Adoption is not issued, coverage shall not be continued for such Adopted child. As the Contractholder, you are solely responsible for notifying us and the Marketplace if the Adoption does not take place. Upon receipt of notification from the Marketplace, we will terminate the coverage of the child on the first billing date following receipt of the written notice.
Adopted Children. An adopted Dependent Child may be enrolled by Employee within either:
Adopted Children. To enroll an adopted child (other than a newborn), you must submit to us any required application or enrollment forms along with proof of the adoption and any additional Premium, within 60 days after the date of placement.
1) If the adopted child is enrolled within 30 days, no additional Premium will be charged for the child for the first 30 days of the child’s coverage.
2) If the adopted child is enrolled within 31 to 60 days, the Premium will be charged from the moment of placement. You must pay the additional Premium for coverage to be provided to the adopted child.
3) The Effective Date of coverage for an adopted child (other than an adopted newborn child) shall be the date such adopted child is placed in the Contractholder’s residence pursuant to Florida law, or the date of adoption, whichever is earlier. However, coverage for such child shall not be required in the event that the child is not ultimately placed in the Contractholder’s residence, in compliance with Chapter 63, Florida Statutes.
Adopted Children. Benefits will be provided to a Subscriber’s adopted child, if such child is placed prior to his or her 18th birthday in compliance with Chapter 63, Florida Statutes.

When a Member is transferred outside of the region, the Contractor will continue to be responsible for arranging care for the Member until removed from the HIPAA 834. The DCBS shall notify the Department when a Member’s case is transferred outside the Contractor’s Region. The Department will include notice of the transfer in the HIPAA 834.
Adopted Children. If the Subscriber has a new Dependent as a result of adoption, coverage for the new Dependent is automatically provided from the earlier of the date of placement for the purpose of adoption or the date of the entry of an order granting you custody of the child for purposes of adoption, for up to 31 days. If coverage was issued outside the Indiana Marketplace, coverage for the new Dependent shall continue beyond the initial 31 day period provided the Subscriber submits a form to us to add the new Dependent under this Contract and we receive payment for additional applicable premium, in any, within the initial 31 days. The Effective Date will be the earlier of the date of adoption or placement of adoption. If coverage was issued inside the Indiana Marketplace, coverage for the new Dependent shall continue beyond the initial 31 day period provided the Subscriber requests a Special Enrollment Period through the Indiana Marketplace to add the new Dependent to this Contract. The Special Enrollment Period request must be submitted to the Indiana Marketplace within 60 days from the date of adoption or placement of adoption. We must receive payment for additional applicable premium, in any, within 30 from the date we receive the new Dependent’s enrollment transaction from the Indiana Marketplace. The Effective Date will be the earlier of the date of adoption or placement of adoption.
