Common use of Application Form Clause in Contracts

Application Form. An application for additional temporary income shall be: i) on a form approved by the Superintendent; ii) where the Annuitant is a former member of a pension plan, accompanied by the written consent of the Spouse of the Annuitant; and iii) submitted to the Trustee at the beginning of the Fiscal Year of the Fund, unless otherwise permitted by the Trustee.

Application Form.
