ARTICLE DENTAL PLAN. For Full Time Employees The Employer shall pay seventy-five percent (75%) of the premium costs for a basic dental plan similar or the same as the Blue Cross Plan plus Rider current rates, if the requirements of the Insurance Company are met. The employer shall pay seventy-five percent (75%) of the premium costs for additional dental benefits or the same as the Blue Cross Rider Level IV, providing orthodontics for each insured child between six (6) and eighteen (18) years of age, providing co- insurance, to a lifetime maximum of if the requirements of the insurance company are met. The employer shall pay seventy-five percent (75%) of the premium costs for additional dental benefits or the same as the Blue Cross Rider Level providing the creation and repair of crowns and bridges, providing co-insurance, to a annual maximum per insured employee and dependant, if the requirements of the insurance company are met. Note: Co-insurance means that one half of the "out of pocket" expenses under the current Ontario Dental Association rates are reimbursed up to the specified maximum.
ARTICLE DENTAL PLAN a) The Company agrees to contribute a maximum of twenty-three (23) cents per hour effective December 1997; a maximum of twenty-four (24) cents per hour, effective December 1998; and a maximum of twenty-five (25) cents per hour, effective December for all hours paid and worked to the Trustees Dental Plan Ontario (“dental plan”).
