ARTICLE DENTAL PLAN a) The Company agrees to contribute a maximum of twenty-three (23) cents per hour effective December 1997; a maximum of twenty-four (24) cents per hour, effective December 1998; and a maximum of twenty-five (25) cents per hour, effective December for all hours paid and worked to the Trustees Dental Plan Ontario (“dental plan”). Hours paid and worked is defined as regular hours, vacation, paid holidays, and sick days, to a maximum of hours per week per employee, but shall not include overtime or weekly indemnity payments. The Company is a signatory to a “Participation Agreement” and will supply any documents, forms, reports or information required by the Trustees of the dental plan. The Company shall forward all contributions, together with a list of all employees and the number of hours paid and worked for each employee in each reporting period, within fifteen (15) days following the end of each of the Company’s accounting periods.
ARTICLE DENTAL PLAN. For Full Time Employees The Employer shall pay seventy-five percent (75%) of the premium costs for a basic dental plan similar or the same as the Blue Cross Plan plus Rider current rates, if the requirements of the Insurance Company are met. The employer shall pay seventy-five percent (75%) of the premium costs for additional dental benefits or the same as the Blue Cross Rider Level IV, providing orthodontics for each insured child between six (6) and eighteen (18) years of age, providing co- insurance, to a lifetime maximum of if the requirements of the insurance company are met. The employer shall pay seventy-five percent (75%) of the premium costs for additional dental benefits or the same as the Blue Cross Rider Level providing the creation and repair of crowns and bridges, providing co-insurance, to a annual maximum per insured employee and dependant, if the requirements of the insurance company are met. Note: Co-insurance means that one half of the "out of pocket" expenses under the current Ontario Dental Association rates are reimbursed up to the specified maximum.
