ARTICLE DENTAL PLAN. For Full Time Employees The Employer shall pay seventy-five percent (75%) of the premium costs for a basic dental plan similar or the same as the Blue Cross Plan plus Rider current rates, if the requirements of the Insurance Company are met. The employer shall pay seventy-five percent (75%) of the premium costs for additional dental benefits or the same as the Blue Cross Rider Level IV, providing orthodontics for each insured child between six (6) and eighteen (18) years of age, providing co- insurance, to a lifetime maximum of if the requirements of the insurance company are met. The employer shall pay seventy-five percent (75%) of the premium costs for additional dental benefits or the same as the Blue Cross Rider Level providing the creation and repair of crowns and bridges, providing co-insurance, to a annual maximum per insured employee and dependant, if the requirements of the insurance company are met. Note: Co-insurance means that one half of the "out of pocket" expenses under the current Ontario Dental Association rates are reimbursed up to the specified maximum.
ARTICLE DENTAL PLAN a) The Company agrees to contribute a maximum of twenty-three (23) cents per hour effective December 1997; a maximum of twenty-four (24) cents per hour, effective December 1998; and a maximum of twenty-five (25) cents per hour, effective December for all hours paid and worked to the Trustees Dental Plan Ontario (“dental plan”). Hours paid and worked is defined as regular hours, vacation, paid holidays, and sick days, to a maximum of hours per week per employee, but shall not include overtime or weekly indemnity payments. The Company is a signatory to a “Participation Agreement” and will supply any documents, forms, reports or information required by the Trustees of the dental plan. The Company shall forward all contributions, together with a list of all employees and the number of hours paid and worked for each employee in each reporting period, within fifteen (15) days following the end of each of the Company’s accounting periods.
ARTICLE DENTAL PLAN. Effective the first of the month following the date of ratification of this agreement and limited to dental work performed on and after that date, the basis for payment for covered services shall be the current Manitoba Dental Association Fee Guide; The current Fee Guides will be implemented effective January Iof each respective year. Dental coverage will continue for the first seventeen (17) weeks of Maternity Leave effective the first of the month following the date of ratification and limited to maternity leaves commencing on and after that date The annual maximum per claimant will be as follows: effective January one thousand, four hundred and seventy-five dollars ($1,475); effective January one thousand, five hundred seventy-five dollars ($1,575); effective January one thousand, six hundred and seventy-five dollars ($1,675). The orthodontic lifetime maximum will be as follows: effective January one thousand six hundred dollars ($1,600); effective January one thousand six hundred seventy-five dollars ($1,675); Pad-time employees will be eligible for family coverage baaed on fifty percent (50%) of the coverage amounts applicable for full-time employees up to eighty percent (80%) of the maximum.

The Board agrees to pay of the premium cost of a dental plan at the current minus one year Ontario Dental Association schedule of fees with rate changes effective September, coverage effective September The Dental Plan shall be available to all regularly employed Teachers on a voluntary enrolment basis, in accordance with the eligibility requirements of the Plan. Pit and Fissure Sealants. The Board agrees to administer a Pit and Fissure Sealants Plan and pay of the premium. The Board agrees to pay of the premium cost of Orthodontic Services. The Board agrees to pay of the premium cost of Major Restorative Services (i.e. dentures, bridges, caps and crowns).
ARTICLE DENTAL PLAN. The Companies signatory hereto agree to have a Medical Services Association Dental Care Plan in effect by January for employees classified herein who qualify in accordance with the requirements of the Plan and who indicate their desire to be covered by the Plan. The Plan shall provide a maximum of Plan A Basic Services and of Plan B Prosthetic A and Procedures. Employees, their spouses and registered shall be eligible for services under the provisions of the Plan. Effective February Part be added to the Plan. The Company will provide a Dental Plan for all regular full-time employees actively at work upon completion of one (1) year of service. employee covered by some other plan will not be insured in this plan. If at any time, there should be enacted any laws, or regulations, whereby the cost is eliminated as such, or reduced, the contribution of the parties will be affected in a similar manner.
ARTICLE DENTAL PLAN. The Division shall provide a Dental Benefit Plan registered in the of the Division The dental plan w i l l provide basic services coverage equal to or better than Level I of the Cross Dental Plan. Premiums for the dental plan w i l l paid by the Division and the annual premium cost w i l l be fully recovered by reducing the salary schedule as of July by the same dollar amount for all employees. Any future changes in premium costs of the plan w i l l also be reflected by a uniform dollar change in the salary schedule at an established annual date. The plan w i l l be compulsory for all eligible teachers. The Division w i l l implement the plan as soon as possible after signing of the collective agreement. Copies of all division correspondence with the dental plan carrier shall be forwarded to the
