The declaration must be signed and dated by the exporter or producer or an authorised representative of the exporter or producer accompanied by the following statement: I declare that the good(s) described in this document qualify as originating in [NAME OF PACER PLUS PARTY(IES)] in accordance with the provisions of Chapter 3 (Rules of Origin and Verification Procedures) of the Pacific Agreement on Closer Economic Relations Plus (PACER Plus) and the information contained in this document is true and accurate. I assume responsibility for proving such representations and agree to maintain for a period of five years and present upon request or to make available during a verification visit, documentation necessary to support this declaration.
