To accompany all Change Orders, the Contractor shall furnish a breakdown of expenditures for labor and materials by units and quantities in the form prescribed by the Owner, and the breakdown shall be accompanied by the following declaration: “I do solemnly swear, under criminal penalty, that the costs shown hereinabove do not exceed current costs for like services or materials and do not exceed the actual costs to the Contractor therefor, and that the quantities shown do not exceed actual requirements.” For all force account changes, the Contractor shall promptly, and in no event later than thirty (30) days after receipt of written demand therefor, pursuant to Article 29(i) submit to the Engineer a complete, accurate, and final breakdown and account together with vouchers, showing all expenditures and percentages allowable under Case (c). For all unit price changes, the Contractor shall promptly, and in no event later than thirty

The CMR shall furnish to the Board and the Design Professional an itemized breakdown of the quantities and prices and expenditures for labor and materials used in computing the proposed change in Contract Sum, in a format acceptable by the Board, which shall contain the following elements:
(a) Restatement of the scope of work of the proposed Change Order.
(b) Itemized breakdown of items, quantities, unit costs and total costs; application of overhead and profit amounts, and grand total.
(c) Statement of the CMR in the following exact language:
(d) Signature and title of the person submitting the breakdown of expenditures for the CMR.
(e) Signature and date of the Contract Compliance Specialist certifying to the following statement to be inserted below the signature of the CMR in the following exact language:

For all force account changes the Construction Manager shall promptly and in no event later than thirty (30) days after receipt of written demand therefore pursuant to this Part, submit to the Architect a complete, accurate, and final breakdown and account together with vouchers, showing all expenditures and percentages allowable under Case (c). For all unit price changes the Construction Manager shall promptly and in no event later than thirty (30) days after receipt of written demand therefore pursuant to this Part submit to the Architect and the Program Manager an accurate account of the quantity of work performed under Case (b). In any case, the Architect shall certify to the amount [including under Case
(a) and Case (c) the allowance prescribed in the Contract Documents for overhead and profit] due the Construction Manager. The Construction Manager shall obtain and furnish as back up to the Construction Manager‟s breakdown a separate breakdown for each Trade Contractor's charges prepared by each Trade Contractor on the letterhead of the Trade Contractor and properly signed by the Trade Contractor.
