HEBP shall generate Certificates of Creditable Coverage for all Participants terminating group health coverage provided by the Plan, including coverage held under the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA), as required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). The documents will provide the period of coverage beginning on the Participant’s date of enrollment in the Plan (if provided by the Plan Administrator as specified under SECTION III – DUTIES AND RESPONSIBILITIES OF PLAN ADMINISTRATOR, 3.02 of this Agreement) and ending with the Participant’s date of termination as verified by eligibility records provided by Plan Administrator and maintained by HEBP. These Certificates of Creditable Coverage will be mailed to the Participant’s last known address as provided to HEBP by the Plan Administrator. If such address is not made available to HEBP or is unavailable, the Certificate of Coverage will be sent to the Plan Administrator for appropriate delivery.
