Certification of Serious Health Conditions. When requesting Family and Medical Leave for serious health conditions, an employee must provide certification from a health care provider which includes: 1. the date on which the serious health condition commenced; 2. the probable duration of the condition; 3. any appropriate medical facts; 4. a statement containing specific information why the employee is needed to care for the child, spouse, or parent, or; a statement containing specific information why the employee is unable to perform the functions of the job;
