Claim Filing Process. Settlement Class Members shall be permitted to make a Claim for a Claim Settlement Payment by submitting a claim on the Settlement Website on a date no later than the Claim Deadline. Any Settlement Class Member who does not submit an accurate and fully completed Claim Form by the Claim Deadline shall be deemed to have waived any Claim and any such Claim will be rejected.

Except for Members of the Collection Letter Subclass who are also members of the Credit Reporting Subclass, any Credit Reporting Subclass Member who does not mail or email a completed Claim Form by the Claim Deadline shall be deemed to have waived any claim to relief and any such untimely Claim Form will be rejected. All Members of the Collection Letter Subclass who are also Credit Reporting Subclass Members shall be entitled to the credit reporting relief under this Agreement, regardless of whether they submitted a Claim Form or of any inaccuracy or deficiency in any Claim Form they may submit.

A written Claim Form will also be available on the Settlement Website for Settlement Class Members to download or print out and mail to the Settlement Administrator pursuant to this Section; 2) Settlement Class Members will be permitted to upload and file completed Claims Forms through the website; and 3) Settlement Class Members shall also have the option of completing their Claim Form online within the settlement website, utilizing an e- signature format; provided however, for those Claims requiring verification documents, the Claimant must upload scanned copies of those verification documents to the settlement website, with the appropriate claim number to associate the upload with the Claim. Any Settlement Class Member who does not mail, properly upload or submit online, a completed Claim Form by the Claim Deadline shall be deemed to have waived any claim to Claim Settlement Relief and any such Claim Settlement Form will be rejected.

Claim Filing Process. RoundPoint Settlement Class Members shall be permitted to make a Claim in any one of three ways: (a) By mailing to the Settlement Administrator (either through posting with the United States Postal Service or through a private mail carrier, such as UPS or Federal Express, provided that proof of the mail date is reflected on the label of the mailing) a completed Claim Form and identify verification document(s), providing the information and affirmations required in Section 7.1 above, postmarked no later than the Claim Deadline. A blank Claim Form will also be available on the Settlement Website for RoundPoint Settlement Class Members to print and mail to the Settlement Administrator; (b) By uploading a completed Claim Form and electronic versions of identity verification document(s) through the Settlement Website by midnight Eastern Time (“ET”) on the Claim Deadline, identifying the correct claim number to associate with the uploaded documents; or (c) By completing and submitting a Claim Form online on the Settlement Website by midnight ET on the Claim Deadline, utilizing an e-signature format, and uploading electronic versions of identity verification document(s) on the Settlement Website, identifying the correct claim number to associate with the uploaded documents. Any RoundPoint Settlement Class Member, whether they received the Mail Notice of the Settlement or not, and whether they had knowledge of the Settlement or not, who does not mail, properly upload or submit online through the Settlement Website a completed Claim Form by the Claim Deadline, with verification documents, shall be deemed to have waived any claim to Claim Settlement Relief and any such Claim Form will be rejected, and shall be deemed to have forever released, relinquished, and discharged each of the Released Parties from any and all Released Claims, including but not limited to all claims arising out of, relating to, or in connection with the initiation, settlement, prosecution, or dismissal of the Litigation.
