CO-SIGNER. (check the appropriate box) ☐ - This Agreement shall not have a Co-Signer. ☐ - This Agreement shall have a Co-Signer known as __________________ hereinafter known as the "Co-Signer” and agrees to the liabilities and obligations on behalf of the Debtor under the terms of this Agreement. If the Debtor does not make a payment, the Co-Signer shall be personally responsible and therefore is guaranteeing payment of the principal, late fees, and all accrued interest under the terms of this Agreement.

CO-SIGNER. The parties agree there is no co-signer for this Note.

CO-SIGNER. By affixing signature below, co-signer promises to guarantee the Tenants compliance with the financial obligation of the Rental Agreement. Co-signer understands that he/she may be required to pay: current rent, past due rent, collection costs, non- sufficient funds charges, court costs, late fees, lease fees, advertising costs, cleaning, repairs, or costs that exceed Tenant’s security deposit. Co-signer further agrees that ▇▇▇▇▇▇▇▇ will have no obligation to report to Co-signer should Tenant fail to abide by the terms of the Rental Agreement and waives presentment, demand, protest and notice of acceptance, notice of demand, notice of protest, notice of dishonor, notice of default, notice of nonpayment, and all other notices to which co-signer might otherwise be entitled. Co-signer recognizes that ▇▇▇▇▇▇▇▇ has agreed to rent to Tenant only because of this guaranty and that the continued validity of this guaranty is a material term of this Rental Agreement. Co-signer further understands that if Landlord and Co-signer are involved in any legal proceeding arising out of this Rental Agreement, the prevailing party shall recover reasonable attorney fees, court costs and any cost reasonably necessary to collect a judgment. Co-signer understands that this will remain in force through the entire term of the Tenant’s tenancy, even if their tenancy is extended/or changed in its terms. The following items are required to remove a co-signer from a renewal Rental Agreement: a. Co-signer must remain liable under this Rental Agreement for a minimum of one year; or initial term of lease if shorter than 1 year. b. There can be no late rent payments; c. No disconnect notices from any of the utility companies; d. No lease violations during the lease period; e. There can be no balance owing on the account; f. Frontline Property Management must do a property inspection to confirm that the property is properly maintained; and supervisor approval in writing releasing Co-signer from liability under the Rental Agreement.

CO-SIGNER. Each co-signer to this Agreement is a parent or legal guardian of the Student. Each co- signer shall be jointly and severally liable for the obligations of the Student under this Agreement. This Agreement shall not be effective as against the University unless and until this Agreement is signed by each parent or legal guardian of the Student.

CO-SIGNER. In the event Lessee defaults on any obligations under the terms of said Lease, all obligations herein transfer to Co-signer and or his or her assigns.

CO-SIGNER. 1. Party B’s legal representative agrees to be Party B’s co-signer and guarantees Party B’s performance of its obligations stated in the respective clauses of the Lease Agreement. If Party B breaches the Lease Agreement, co-signer will take joint liability and fulfills the obligations which would have been performed by Party B. 2. In of the event of change of Party B’s aforesaid legal representative after the execution of the Lease Agreement, Party B shall promptly notify Party A and proceed to change co-signer. However, prior to completion of affecting the change of cosigner, the original legal representative shall still bear the aforesaid liability, which shall not be waived until the change is affected.

CO-SIGNER. If a member is under 18 years of age at the date the terms and conditions of this Agreement are accepted, a Co-signer is required. As Swing Zone would not have agreed to enter into this Agreement without the promises of the Co-signer, the Co-signer promises to pay all amounts due to Swing Zone under this Agreement when due, and to keep and perform all the member's promises and agreements. The Co- signer’s promises and agreements are joint and several with the member's promises and agreements. If more than one person signs this Agreement as Co-signer, the promises are both joint and several.

CO-SIGNER. Where this Agreement has been signed by a co-signer and/or when personal training is purchased as a gift, the co- signer agrees, as primary debtor and not merely as guarantor or surety, to pay all amounts required to be paid to us as and when due and to otherwise perform all of the obligations of the Client under this Agreement. The liability of the Client and the co-signer under this Agreement shall be joint and several. Co-signer must be a legal guardian/parent over age 25 for any member below age 19 (yet meets our age requirements).
