These combined forms are only available for public works projects under $2,500 including tax and limited public works projects under $35,000 including tax. b) The combined form may only be used if one payment will be made under the contract and if there are no subcontractors. If the project involves subcontractors or multiple payments, separate statements and affidavits must be filed.
