Complaint Intake Report. When a complaint regarding attorney performance is received by the staff of the COMMITTEE or by the CONTRACTOR, an Intake Report shall be completed, which Report appears as Attachment 2 to this Contract. Additionally, the CONTRACTOR shall enter the Complaint information in the CPCS Complaint Database, unless the Program has already done so.
