Comprehensive Medical. DENTALCOVERAGE
Section 28.1 The City shall provide comprehensive group medical coverage to each full-time covered employee with a minimum lifetime maximum of $1,000,000.00 per participant.
Section 28.2 The City shall provide single dental insurance and shall make available dependent coverage. The employee shall pay the additional cost for dependent coverage.
Section 28.3 Effective December 1, 2019, employees with a base salary equal to or less than $40,000 shall pay ten percent (10%) of the City's total monthly premium cost for medical and single dental insurance. Employees with a base salary greater than or equal to $40,000 shall pay no more than fourteen (14%) of the City's total monthly premium cost for medical and single dental insurance.
Section 28.4 The Union recognizes the right of the City in its discretion to secure alternate insurance carriers and to modify coverages which measures may be used to maintain or to lessen premium costs. Prior to any modifications of benefits or coverage, the Union and the City agree to meet and discuss any such modifications. During the life of this contract, the offered medical and dental plan terms for this bargaining unit shall be equal to the terms for all other City employees.
Section 28.5 Union representatives are welcome to meet with the City through the Human Resources Director in order to maintain communication between the City and employees regarding health insurance, (medical and dental insurance) and wellness. The Union representatives will then assist in the dissemination of the information to the members of the bargaining unit.

(e) Orthopedic shoes (f) Dental (90% refundable) - Crowns, inlays, bridges and dentures, subject to 50% co- insurance with unlimited maximum. Orthodontics subject to 50% co- insurance with a $2,000.00 lifetime maximum for employee, spouse and each dependent.
Comprehensive Medical. A. Effective January 1, 1998, the Board shall pay the following percentage of its share of the cost of Medical Insurance for all employees hired after December 31, 1997. Bus Drivers (excluding single route or shuttle drivers) – one hundred percent (100%) of the Board’s share 0-14 regular scheduled hours – not eligible 15-24 regular scheduled hours – fifty percent (50%) of the Board’s share 25-40 regular scheduled hours – one hundred percent (100%) of the Board’s share Grandfather language: For the 2021-2022 school year through the 2023-2024 school year, the Board shall pay the Board Single and the Board Family premium for any employee hired on or before December 31, 1997 regardless of hours.
B. The Board shall pay the cost of comprehensive major medical program coverage for each member of the bargaining unit with the employee paying a monthly contribution toward the cost of this program in the amount of: Single Family Employee Employee 2021-2022 $ 72.00/month $144.00/month 2022-2023 $ 77.00/month $154.00/month 2023-2024 $ 87.00/month $174.00/month
C. In any year that the District receives premium holiday(s), the District will waive the monthly employee’s premium contribution for the same number of months as the number of premium holidays granted in that year.
D. In cases where husband and wife are employed in the District, they are entitled to either two (2) single policies or one (1) family policy. The terms and conditions of the policy coverage shall provide Employees coverage which is no less than that provided to other Employees of the District. These coverages shall include at a minimum: Overall Lifetime Maximum Benefit Unlimited Deductible Amount: Individual $250/$500 Family $500/$1,000 Co-Payment (after Deductible is satisfied)90/10 in-network and 80/20 out of network Max Out-of-Pocket Individual $1,000/$2,000 Family $2,000/$4,000 The Board also agrees to make optional coverage available to Employees which would provide coverage through a H.M.O., P.P.O, etc.
