Common use of Designation of Claim Clause in Contracts

Designation of Claim. Upon receipt of a pre-service claim, The Plan will make a determination if the claim involves Urgent Care. If a Physician with knowledge of the Member’s medical condition determines the claim involves Urgent Care, The Plan will treat the claim as an Urgent Care claim.
