Documentation of Attendance. All Contractors providing office- or clinic-based services, including case management, must maintain documentation of the client's attendance at the visit or session. This requirement is in addition to the required progress notes by which the clinician (e.g. physician/physician extender, nurse, dentist, nutritionist, social worker, therapist, case manager, physical therapist) or staff member (e.g. food pantry worker) documents the service provided. Subrecipient must implement a strategy that ensures clients receiving office- or clinic-based care or services sign-in when they access such services. In specific situations where a client's mental health status, behavior and/or life situation may present challenges in obtaining signatures or otherwise documenting the client checking-in at the agency site, Subrecipient may forego this requirement if obtaining the signature may discourage the client from accessing needed care or services. This is only on a case-by-case basis and must be noted in the client record.
