Documentation Requirement. Seller shall provide to Company (i) the manufacturer's recommended schedule for the calibration and maintenance of each component of the Monitoring and Communication Equipment and (ii) subject to the limitation set forth in Section 1(a)(ii) (As-Builts) of Attachment B (Facility Owned by Seller) of this Agreement, documentation of the performance of all such calibration and maintenance per manufacturer specifications. Although Company is to receive from Seller the aforesaid recommended schedules for calibration and maintenance, as well documentation of the performance of all such calibration and maintenance, Company shall have no responsibility to monitor Seller's compliance with such calibration and maintenance schedules. Accordingly, any failure by Company to bring Seller's attention any apparent failure by Seller to perform such recommended calibration and maintenance shall neither relieve Seller of its obligations under this Agreement to perform such calibration and maintenance nor constitute a waiver of Company's rights under this Agreement with respect to such failure in performance by Seller.
Documentation Requirement. Each ves- sel covered by an operating agreement (includ- ing an agreement terminated under section 53104(c)(2)) shall remain documented under chap- ter 121 of this title, until the date the operating agreement would terminate according to its terms.
Documentation Requirement. Business Associate shall maintain written records relating to (i) each Breach of Unsecured PHI and (ii) each suspected Breach which is later determined to not constitute a Breach of Unsecured PHI for a period of the lesser of six (6) years or the duration of this Agreement. Business Associate shall maintain records relating to actual or suspected Breaches (even if it is determined that no notice is required under the Breach Notification Rules), including all risk assessments for determining risk of harm to affected individuals and all analyses of whether the Breach Notification Rules are implicated by an actual or suspected Breach. Business Associate shall also maintain all records relating to actions taken in response to a Breach of Unsecured PHI, including all notices provided in accordance with the Breach Notification Rule, all steps to mitigate harm caused by the Breach and all corrective action steps taken to prevent a future similar Breach. Upon termination of the Agreement, Business Associate shall provide to Covered Entity all such documentation for the previous six (6) year period (or for the period in which this section is in effect, if shorter).

Documentation Requirement. An employee on sick leave for more than three (3) consecutive days may be required to furnish a doctor’s certificate prior to returning to work. In any case if the Sheriff has reasonable grounds to believe sick leave is being abused, the employee may be required to furnish a doctor’s verification of illness. Any employee out ill for thirty (30) consecutive calendar days shall provide the Sheriff with a doctor’s verification of illness. Such verification shall be submitted every thirty (30) consecutive days thereafter. If the employee fails to submit sufficient proof of illness when required to do so, such absence shall be considered as time off without pay. Upon return from sick leave of twenty (20) calendar days or more, the employee shall submit to the Sheriff a physician’s statement attesting to the employee’s recovery and physical fitness to perform the duties of his/her assignment. No employee will be allowed to return to work who has failed to present such physician’s statement. The County shall have the right at its discretion to verify the report of the attending physician concerning the illness or disability of an employee, and to require the employee to be examined, at the County’s expense, by a physician selected by the County to determine the nature and extent of the illness or disability.

In addition to any other information that ALICOMP may request regarding Software and Computer Equipment for which COSI requests written consent, COSI shall provide access to ALICOMP to all technical manuals and other documentation for such Software and Computer Equipment in such reasonable amounts as ALICOMP may request from time to time.
