DRUG SCREENING and BACKGROUND CHECKS. I understand before I can participate in clinical training, I must have a Level II background check by Student Check. I understand that information received regarding any offense will be handled on an individual basis. I understand an offense of a serious nature could result in the following: • unable to attend the required clinical training for the program. • not be permitted to take the Florida State Board Examination, which is mandatory to obtain a practical nursing license. • not be employed after completion of the program. I understand that prior to clinical training; a urine drug screen will be required. I understand that I will be contacted by the Medical Review Officer should the test results indicate a positive reading for illegal substances or prescription drugs. If the substance is illegal or a drug for which I do not have a valid prescription, I will be immediately dismissed from the Practical Nursing program.

I understand that information received regarding a FDLE offense will be handled on an individual basis. I understand an offense of a serious nature could result in the following. • I may be unable to attend the required clinical training for the program. • I may not be permitted to take the RHS, ICE and AMP portions of the XXXXX. • I may not be employed after completion of the program. I understand that a urine drug screen will be required. I understand that I will be contacted by the Medical Review Officer should the test results indicate a positive reading for illegal substances or prescription drugs. If the substance is illegal or a drug that I do not have a valid prescription for, I will be immediately dismissed from the Medical Assisting program.
DRUG SCREENING and BACKGROUND CHECKS. To the extent requested by Buyer, required by Applicable Law, or otherwise indicated by applicable good industry standards, Seller will ensure that all individuals within the Seller Group are drug tested and subject to a background investigation prior to first reporting to any Site. Seller will not allow any individual within the Seller Group to perform work related to Goods and Services where the results of any drug test are above the generally accepted Substance Abuse & Mental Health Services Administration limits as allowed by state law, or a background investigation uncovers results that are of reasonable concern. The drug screen and background check are to be performed at the direction of the Seller and at its expense. When on Site, Seller warrants that Seller Group will maintain a drug-free workplace, and will comply with Site-specific rules regarding safety and health matters. As such, all individuals within the Seller group may be removed from the Site for possession and/or suspected use of a prohibited substance. The Buyer reserves the right to audit Seller Group practices to ensure compliance with the requirements described above.

The information received regarding any offense will be handled on an individual basis and an offense of a serious nature could result in the following inabilities: • attend the required clinical training for the program, • take the Florida State Board Examination, which is mandatory to obtain a practical nursing license, and/or • be employed after completion of the program. In addition, a urine drug screen will be required by Student Check (information will be given the first day of class). The student will be contacted by the Medical Review Officer should the test results indicate a positive reading for illegal substances or prescription drugs. If the substance is illegal, or the student does not have a valid prescription for the drug detected, the student will be immediately dismissed from the Practical Nursing Program.

  • Background Screening VENDOR shall comply with all requirements of Sections 1012.32 and 1012.465, Florida Statutes, and all of its personnel who (1) are to be permitted access to school grounds when students are present, (2) will have direct contact with students, or (3) have access or control of school funds, will successfully complete the background screening required by the referenced statutes and meet the standards established by the statutes. This background screening will be conducted by SBBC in advance of VENDOR or its personnel providing any services under the conditions described in the previous sentence. VENDOR shall bear the cost of acquiring the background screening required by Section 1012.32, Florida Statutes, and any fee imposed by the Florida Department of Law Enforcement to maintain the fingerprints provided with respect to VENDOR and its personnel. The parties agree that the failure of VENDOR to perform any of the duties described in this section shall constitute a material breach of this Agreement entitling SBBC to terminate immediately with no further responsibilities or duties to perform under this Agreement. VENDOR agrees to indemnify and hold harmless SBBC, its officers and employees from any liability in the form of physical or mental injury, death or property damage resulting from VENDOR’s failure to comply with the requirements of this section or with Sections 1012.32 and 1012.465, Florida Statutes.

  • DRUG AND ALCOHOL TESTING The employer reserves the right to introduce drug and alcohol testing during the life of this agreement. Prior to introduction of testing there will be reasonable consultation with the employees.

  • Drug Testing (A) The state and the PBA agree to drug testing of employees in accordance with section 112.0455, F.S., the Drug-Free Workplace Act.
