Effective Date of Coverage. An eligible employee is entitled to benefits provided he is actively at work on the first day the Long Term Disability Benefit Plan becomes effective. An eligible employee absent from work due to sickness or accident at the effective date of the Plan, shall only be eligible for Long Term Disability Plan benefits upon the return to continuous active full-time employment for a period of more than four consecutive weeks. The Company shall have the right to give medical examinations to employees returning from such lay-off to determine their eligibility under the Plan.

Effective Date of Coverage. An individual may apply for coverage under this Contract through the Company and may include any eligible Dependents in such application. No person for whom coverage is sought will be covered under this Contract unless the application for coverage has been approved by the Company and such approval has been evidenced by the issuance of an identification card or other written notice of approval. Payment of premiums to the Company for any person for whom coverage is sought will not effectuate coverage unless and until the Company’s identification card or other written approval has been issued, and in the absence of such issuance, the Company’s liability will be limited to refund of the amount of premiums paid. When an application has been approved and any premiums for coverage have been paid in advance as required by this Contract, coverage will commence on the date the Company assigns as Your Effective Date. No Claims will be paid for dates of service prior to Your Effective Date.

Inpatient Hospitalization and Enrollment: MSHO and MSC+ Enrollees receiving Inpatient Hospitalization will be enrolled in accordance with section 3.1.5(A) above. All charges related to Inpatient Hospitalization for any Enrollee on the effective date of enrollment will not be the responsibility of the MCO. MCO coverage will begin the day following discharge from the hospital. MCO coverage under MSC+ for Medical Assistance Beneficiaries who disenroll from MSHO and are required to remain enrolled in MSC+ but who are hospitalized on the first effective date of re-enrollment in MSC+ shall commence according to 3.1.5(B)(1) above. Maintenance of Enrollment Forms. Original enrollment forms will be maintained by the STATE, MCO or the Local Agency, whichever enrolled the Enrollee, and may be imaged in accordance with Minnesota Statutes, § 15.17. Enrollee Eligibility Review Dates. In accordance with Minnesota Statutes, § 256.962, subd.8, the STATE will provide a report of eligibility review dates for Enrollees covered under this Contract and enrolled in the MCO.
Effective Date of Coverage. If a Member is an inpatient in a Hospital on the Effective Date of Coverage, the Member will be covered as of that date. Such services are not covered if the Member is covered by another health plan on that date and the other health plan is responsible for the cost of the services. HMO will not cover any service that is not a Covered Benefit under this Certificate. To be covered, the Member must utilize Participating Providers and is subject to all the terms and conditions of this Certificate.

Effective Date of Coverage. Coverage becomes effective for an employee on the date the employee becomes eligible for coverage as set out in the policy. If an employee is absent from work because of a disability due to illness or injury on the date the employee's coverage (or an increase in coverage) would otherwise become effective, such coverage (or increased coverage) will not become effective until the date the employee returns to active work for one full day.
