Electronic Protected Health Information. “Electronic Protected Health Information” (“EPHI”) means individually identifiable health information that is transmitted or maintained in electronic media, limited to the information created, received, maintained or transmitted by Business Associate from or on behalf of Covered Entity.

Electronic Protected Health Information. “Electronic Protected Health Information” means individually identifiable health information that is transmitted by or maintained in electronic media.

Electronic Protected Health Information or Electronic PHI means protected health information that is transmitted by electronic media (as defined by the Privacy and Security Regulations) or is maintained in electronic media. Electronic PHI may be transmitted and maintained on devices such as cell phones, PDAs, text pagers, and USB static discs.

Electronic Protected Health Information. “Electronic Protected Health Information” shall have the same meaning as the term “Electronic Protected Health Information” in 45 C.F.R. § 160.103, and shall refer to PHI that Business Associate, or Subcontractor on behalf of Business Associate, transmits or maintains in electronic media.
Electronic Protected Health Information. BCBSRI will comply with the Security Rule (45 C.F.R. Part 164, Subpart C) with respect to the Plan’s PHI that BCBSRI maintains or transmits in electronic format.
Electronic Protected Health Information. To the extent that Business Associate creates, receives, maintains or transmits Electronic Protected Health Information on behalf of Covered Entity on or after April 21, 2005, Business Associate agrees to: A. Implement Administrative, Physical and Technical Safeguards that reasonably and appropriately protect the confidentiality, integrity and availability of the Electronic Protected Health Information; B. Ensure that any agent, including a subcontractor, to whom it provides Electronic Protected Health Information agrees to implement reasonable and appropriate safeguards to protect it; and C. Report to Covered Entity any Security Incident of which Business Associate becomes aware.
