Enhanced Benefits Report. The Health Plan shall submit a monthly report of all claims paid for the following procedure codes in the prescribed format below. The report shall be submitted to the Bureau of Health Systems Development, in the manner and format determined by the Agency, by the 10th Calendar Day of the month for all claims paid for the previous month.
