Common use of Extended Health Clause in Contracts

Extended Health. i. The Board will contribute one hundred per cent (100%) of the premium of the Extended Health Benefits Plan as established by Pacific Blue Cross or its successor. ii. A monthly charge of fifty cents ($0.50) for single coverage and one dollar fifty cents ($1.50) for family coverage shall be added to the costs for Medical Services and shared equally between the employee and the Board to provide additional prosthetic equipment and other items such as heavy-duty wheelchairs, etc. when recommended by a physician. iii.
