FINAL ANNUAL REPORT. The Subrecipient shall furnish the District with a final Annual Report, summarizing findings and activities under this Subaward. A form satisfactory to the District shall be used to submit the final Annual Report, and the report certification must be signed. The report shall be submitted to the District’s Authorized Representative for Business Matters in accordance with the schedule indicated in Exhibit C.
FINAL ANNUAL REPORT. A Final Annual Report covering grant activities over the entire project period must be submitted within 30 days after the end of the project. It must be submitted to OVR via the sharing mechanism in CIL Suite. At a minimum, the following information must be included in the narrative: A. a statement of the progress toward achieving the project’s Deliverables, goals and objectives (see above); B. itemization of how grant funds were utilized, including: 1. the grant staff hired, if applicable; 2. equipment purchased by Grantee, listing equipment and purchase price; and 3. other benefits resulting from the grant; C. description of the major problems and difficulties encountered in the project and their resolution; and D. status of plans for continuing project activities after expiration of grant support for the project.
FINAL ANNUAL REPORT. A copy of the CONSULTING Teacher's final annual report shall be submitted to the REFERRED PARTICIPATING TEACHER and discussed with him/her. The REFERRED P ARTICIPATING Teacher's input and signature will be obtained at least ten (10) days before it is submitted to the JOINT PANEL. The REFERRED PARTICIPATING Teacher's signing of the report does not necessarily mean agreement with its contents, but rather that he/she has received a copy of the report.
