Common use of Formulary Generic Drugs Clause in Contracts

Formulary Generic Drugs. The Subscriber is responsible for paying the Formu- xxxx Generic Drug Copayment for each new and re- fill Formulary Generic Drug prescription. The pharmacist will collect from the Subscriber the Co- payment at the time the Drugs are obtained. If the Plan’s contracted rate for the prescription is less than the Subscriber’s Copayment amount, the Sub- xxxxxxx is responsible for payment of the contracted rate only. The Copayment for Formulary Generic Drugs is shown in the Summary of Benefits.
