Functions and Activities on Covered Entity’s Behalf. Except as otherwise limited in this Agreement, Business Associate is permitted to request the minimum necessary Protected Health Information on Covered Entity’s behalf, and to use and to disclose the minimum necessary Protected Health Information to perform functions, activities, or services for or on behalf of Covered Entity.
Functions and Activities on Covered Entity’s Behalf. For Business Associate to perform the following plan administration, payment, health care operations, and benefit management in conjunction with the services being provided by Business Associate to the Covered Entity:
(A) All services necessary for Business Associate to comply with its obligations under the Parties’ original agreement as well as any and all amendments to the original agreement.
(B) Data aggregation services relating to the health care operations of Covered Entity.
Functions and Activities on Covered Entity’s Behalf. Business Associate may Use or Disclose PHI as necessary to perform functions, activities, or services for or on behalf of Covered Entity, as otherwise permitted in this Agreement or the Service Agreement, or as Required by Law, and will not further Use or Disclose PHI.
Functions and Activities on Covered Entity’s Behalf. Except as otherwise set forth in this Agreement, the parties hereby agree that USI shall be permitted to use and/or disclose Protected Health Information of the Covered Entity only for the purpose of conducting the transactions contemplated under this Agreement and only for purposes within the scope of USI’s representation of the Covered Entity.
4.2.1 (b) Business Operations. USI is permitted to use and/or disclose Protected Health Information, if necessary, for the proper management and administration of USI’s representation of the Covered Entity, or to carry out any legal responsibilities of USI provided that, with respect to any disclosure of Protected Health Information, either:
4.2.1 (b)(1) the disclosure is Required by Law; or
4.2.1 (b)(2) USI obtains reasonable assurances from the person to whom the Protected Health Information is disclosed that: (i) the Protected Health Information will be held in confidence and used or further disclosed only for the purposes for which USI disclosed the Protected Health Information to the person or as Required by Law; (ii) the person will use appropriate safeguards to prevent use or disclosure of the Protected Health Information; and (iii) the person immediately notifies USI of any instance of which it is aware in which the confidentiality of the Protected Health Information has been breached.
4.2.1 (c) Data Aggregation Services. USI is permitted to use or disclose Protected Health Information to provide data aggregation services, as that term is defined by 45 C.F.R. § 164.501, relating to health care operations of the Covered Entity.
Functions and Activities on Covered Entity’s Behalf. Business Associate may use and disclose Protected Health Information to provide services to the Covered Entity. The specific services may be listed in a service agreement, broker of record letter or by other written or verbal agreement. Business Associate is authorized to use Protected Health Information to de-identify it and to use or disclose the de-identified information as agreed to by the parties.
Functions and Activities on Covered Entity’s Behalf. Except as otherwise limited in this BAA, Business Associate may use or disclose PHI to perform functions, activities, services for, or on behalf of, Covered Entity as specified in this BAA or the License Agreement, provided that such use or disclosure would not violate the Privacy Rule if done by Covered Entity. Covered Entity expressly authorizes Business Associate to use or disclose PHI as necessary to request any required authorizations from Covered Entity’s clients sufficient to allow Business Associate to conduct client surveys or engage in other activities necessary to evaluate and improve the operability and functionality of Business Associate’s products.
Functions and Activities on Covered Entity’s Behalf. To provide the following services: third party administrative services for the contracted programs.
Functions and Activities on Covered Entity’s Behalf. Business Associate is permitted to access, use or disclose Protected Health Information to provide services and perform its obligations pursuant to the Agreement. For clarification, Business Associate is not permitted to access, use or disclose Protected Health Information to perform Data Aggregation or to de-identify the Protected Health Information as described at 45 C.F.R. §164.514 or otherwise.
Functions and Activities on Covered Entity’s Behalf. Except as otherwise set forth in this Agreement, the parties hereby agree that Business Associate shall be permitted to use and/or disclose Protected Health Information provided or made available by the Covered Entity (or another business associate of the Covered Entity) only for the purpose of conducting the transactions contemplated for the Business Associate under the underlying Subcontractor Contract and only for purposes within the scope of such underlying Subcontractor Contract.
Functions and Activities on Covered Entity’s Behalf. Agency is permitted to request, use and disclose the minimum necessary Protected Health Information it creates or which it receives from the Covered Entity or on the Covered Entity’s behalf to perform the following functions:
