Functions and Activities on Covered Entity’s Behalf. Except as otherwise limited in this Agreement, Business Associate is permitted to request the minimum necessary Protected Health Information on Covered Entity’s behalf, and to use and to disclose the minimum necessary Protected Health Information to perform functions, activities, or services for or on behalf of Covered Entity.
Functions and Activities on Covered Entity’s Behalf. For Business Associate to perform the following plan administration, payment, health care operations, and benefit management in conjunction with the services being provided by Business Associate to the Covered Entity:

Business Associate is permitted to access, use or disclose Protected Health Information to provide services and perform its obligations pursuant to the Agreement. For clarification, Business Associate is not permitted to access, use or disclose Protected Health Information to perform Data Aggregation or to de-identify the Protected Health Information as described at 00 X.X.X. §164.514 or otherwise.

Except as otherwise limited in this Agreement, to perform functions, activities, or services for, or on behalf of Covered Entity as such services may be specified in any underlying agreement(s), provided that such use or disclosure would not violate 45 C.F.R. Part 164, Subpart E “Privacy of Individually Identifiable Health Information” (the “Privacy Rule”) or 45 C.F.R. Part 164, Subpart C “Security Standards for the Protection of Electronic Protected Health Information” (the “Security Rule”) if done by Covered Entity.

Functions and Activities on Covered Entity’s Behalf. Except as otherwise set forth in this Agreement, the parties hereby agree that USI shall be permitted to use and/or disclose Protected Health Information provided or made available by Covered Entity (or another business associate of Covered Entity) only for the purpose of conducting the transactions contemplated under this Agreement and only for purposes within the scope of USI’s representation of Covered Entity.
Functions and Activities on Covered Entity’s Behalf. Except as otherwise limited in this BAA, Business Associate may use or disclose PHI to perform functions, activities, services for, or on behalf of, Covered Entity as specified in this BAA or the License Agreement, provided that such use or disclosure would not violate the Privacy Rule if done by Covered Entity. Covered Entity expressly authorizes Business Associate to use or disclose PHI as necessary to request any required authorizations from Covered Entity’s clients sufficient to allow Business Associate to conduct client surveys or engage in other activities necessary to evaluate and improve the operability and functionality of Business Associate’s products.
Functions and Activities on Covered Entity’s Behalf. Business Associate is permitted to Use and Disclose PHI it creates or receives for or from Covered Entity or on behalf of Covered Entity, to perform the functions described under the Agreement for the administration of Covered Entity’s health plan.
