Technical Specifications: The technical specifications for measures one and two (M1 and M2), including the denominator and numerator definitions, shall conform to the HEDIS FUH specifications published by NCQA, and shall be applied to the measurement period corresponding to the current Contract Year. The contractor shall report performance as per the 2018 HEDIS measure specification. Within M3(A), the definition of Readmission shall mean the number of episode discharges from a mental health 24-hour Level of Care and the number of discharges that were followed by a subsequent admission to the same or equivalent 24-hour Level of Care within 0-90 days of the discharge date of the episode. Using the count of discharges and the count of readmissions, the rate of readmission is calculated. Within M4, the definition of “arranged prior to discharge” shall mean as evidenced within either the MHS/Connect (the Contractor’s clinical documentation application) or the Provider’s medical record, documentation of: Member and/or family involvement and agreement, the name of the Behavioral Health Covered Service, name of the Provider, and the date and time of the first appointment Measure Tier 2 Goal Tier 1 Goal M1. the rate for 7-day follow-up per HEDIS 2018 specifications greater than or equal to the HEDIS 2017 50th percentile $50,000 greater than or equal to the HEDIS 2017 75th percentile $75,000 M2. the rate for 30-day follow-up per HEDIS 2018 specifications greater than or equal to the HEDIS 2017 50th percentile $50,000 greater than or equal to the HEDIS 2017 75th percentile $75,000 M3(A). Goal Statement. 2.3 Project Kickoff Meeting A project kickoff meeting will be scheduled following issuance of the Notice To Proceed. The project kickoff meeting may include key project staff from ENGINEER and the safety committee, with representatives from FHWA, INDOT, IMPO, and/or other local government partners. The kickoff meeting will include a discussion of the project approach including strategies for safety analysis, equity analysis, public engagement, project selection, and implementation. Other topics will include a communication protocol, project website, dates for key milestones, and a discussion of the federal requirements of the plan. Deliverables:
Goal Statement. The goal of the Transitional Residential Treatment Programs (TRTPs) is to maximize individuals' efforts to achieve the highest possible level of self-sufficiency by implementing or continuing a rehabilitation and recovery process. TRTPs provide a diversion from, and an alternative to, institutional placement such as skilled nursing facilities and local acute hospitals, and promote rehabilitation and recovery from mental health conditions including those that co-occur with substance abuse disorders. Clay Street and Xxxxxx Xxxx House have a separate focus to facilitate collaboration between BHS, the Office of the Conservator, the IMDs (Institutes for Mental Disease), and other social service providers in serving clients who have been confined, some for long periods of time, in locked psychiatric facilities and skilled nursing facilities. Clay Street is wheel chair accessible. Ashbury House has the additional goals of: Family preservation or reunification while providing mental health treatment to mothers who are at risk of losing, or have lost, custody of their children; To facilitate collaboration between BHS UM and Authorization Team, Human Service Agency (HSA), the Department of Public Health (DPH) and other social service providers in serving this special population.
Goal Statement. The Progress Foundation Seniors Program consists of Xxxxxxx House and Rypins House, which are Transitional Residential Treatment Programs (TRTPs). The goals of the program are: To maximize individuals' efforts to achieve the highest possible level of self-sufficiency by continuing the rehabilitation process begun in acute and sub-acute residential programs. To divert as many persons as possible from institutional placements, such as skilled nursing facilities, and "L" facilities, by providing an alternative setting. To reduce recidivism by providing a therapeutic setting in which individuals can grow toward independent living by emphasizing the acquisition and application of survival skills. The development of personal support systems and placement of as many clients as possible in educational, volunteer and vocational or pre-vocational training situations, as well as in jobs in preparation for more independent living.
