Health Center. 1. If you need skilled nursing care, you will have priority access, subject to availability of the appropriate bed, to receive skilled nursing care in the XxXxxx Health Center.
2. You agree to sign a separate Health Center Addendum in its then current form prior to your admission. The terms of this Agreement shall continue to apply, and in the event of a conflict between the terms of this Agreement and the Health Center Addendum, this Agreement shall govern.
3. While receiving skilled nursing care in the Health Center, you will pay a Health Center Room and Board Fee, as may be adjusted from time to time. After you exhaust all of your skilled nursing benefits (under Medicare Part A and/or other insurance plans) you may receive a ten percent (10%) discount on your Health Center Room and Board Fee for the remainder of your stay at the Health Center not to exceed sixty (60) days and limited to one instance. If your stay at the Health Center is not covered by Medicare Part A or other insurance plans, you may receive a ten percent (10%) discount on your Health Center Room and Board Fee not to exceed a total of sixty (60) days. These days or discounts are non-assignable.
a. If you are only receiving skilled nursing care in the Health Center temporarily, you will continue to pay the Monthly Service Fee described in Section V.B., in addition to the Health Center Room and Board Fee.
b. If you transfer permanently for skilled nursing care in the Health Center, and do not wish to relinquish your apartment, you will continue to pay the Monthly Service Fee described in Section V.B., in addition to the Health Center Room and Board Fee.

3. For a temporary stay in the Health Center, you will continue to pay the Monthly Service Fee described in Section V.C. After you exhaust all of your benefit under Medicare Part A – eligible days (and/or other insurance plans) you may receive a 10% discount on your Health Center Room and Board Fee. After you have been a resident of The XxXxxx Village for twelve (12) months, you may be eligible for an additional discount, which is determined annually for CCRC residents. These days or discounts are non-cumulative and non-assignable. The Health Center Room and Board Fee, as adjusted from time to time, will be charged in addition to the Monthly Service Fee.
4. For a permanent stay in the Health Center, if you are not covered for at least 60 days under your Medicare Part A – eligible days (and/or other insurance plans), you may receive a 10% discount on your Health Center Room and Board Fee for 60 days (permanently for CCRC residents who were residents of the apartments as of 10/30/09). If you have been a resident of The XxXxxx Village for twelve (12) months, you may be eligible for an additional discount, which is determined annually for CCRC residents. These days or discounts are non-cumulative and non- assignable.
Health Center. The Community owns and operates a skilled nursing facility (the “Health Center”) located adjacent to the residential areas at Brookhaven. The Health Center is licensed by the Commonwealth of Massachusetts to provide nursing services to persons admitted as residents. (For a description of the Health Center services, see Article V, Section A, 1 of this Agreement.)
Health Center. (Health Center) and (School System) will develop a reporting tool and plan for (Targeted School) School Based Health Center.
Health Center. All incoming full-time students, whether commuting or residential, must submit to the Health Center by August 1, 2012, a completed Health History, Tuberculosis Screening, and Immunization Form signed by a health care provider. Health Center. In 2002, a British organization asked for help from CSRA to start a health center in the health district of Distrito 2. They wanted CSRA to start and manage the health center for the first five years (2002-2007) and then take over the health center themselves. CSRA and the organization signed a five-year contract and started the health center. In 2007, CSRA relinquished control over the health center and told the community of the health center‘s change in management. The community wanted CSRA to continue managing it. Subsequently, community leaders wrote a letter to Xxxxxxx‘s health office to communicate their wishes. In the end, CSRA continued to manage the Distrito 2 Health Center. The Distrito 2 Health Center is the smallest of the three health centers, but it has a doctor and a pharmacy. The health district has a population of 7,000. The residents in the service area of Distrito 2 are mostly cambas like those in Xxxx Roja with a higher socio- economic status and healthier behaviours than in Villa Cochabamba.
Health Center. PROVIDER will supply care in the on-site Health Center when medically necessary without any increase in the then current monthly fee, except for the charge for two extra meals per day, laundry and other expenses not covered in this Agreement. If accommodations are not immediately available in the on-site Health Center, PROVIDER will be responsible to pay for similar services at an off site long term care facility. RESIDENT would be transferred back to the Health Center as soon as accommodations are available.
Health Center. The Health Center is staffed twenty-four (24) hours a day, seven (7) days a week and provides assisted living services, specialized dementia care assisted living services and skilled nursing services (collectively “Health Care Services”).
Health Center. The Health Center is the Community's on-site health care neighborhood, housing the Assisted Living Units, Dementia Care Assisted Living Units and the Skilled Nursing Units. Each distinct area within the Health Center (assisted living, dementia care and skilled nursing) includes a dining room, a resident lounge, activity rooms and bathing facilities.
Health Center i. The entrance door is inaccessible because knob hardware is used. Provide a door with hardware that is easy to grasp with one hand and that does not require tight grasping, pinching, or twisting of the wrist to operate. Lever-operated mechanisms, push-type mechanisms, and U-shaped handles are acceptable designs. Standards § 4.13.9.
ii. The route through the health center is inaccessible because the wall-mounted turkey protrudes 14½ inches into the walkway 76 inches high and would not be detectable to a blind person using a cane. Provide an accessible route at this location such that no objects with their bottom leading edges measuring between 27 inches and 80 inches above the finished floor protrude more than 4 inches into walks, halls, corridors, passageways, or aisles. Free-standing objects mounted on posts or pylons may overhang 12 inches maximum from 27 inches to 80 inches above the ground or finished floor. Standards § 4.
