How to File Claims. 1. A Claim is written or electronic proof that You were charged for services You received while You were covered under this Plan. You must file Claims within 90 days from the date You are admitted to a Hospital, unless it is not reasonably possible to do so. In no event may any Claim be filed later than 15 months from the date services were rendered. We will not pay Benefits if You or Your Providers file a Claim more than 15 months after the date you were admitted to a Hospital.
