Independent Living. (1) What does “independent living unit” mean? The residential units in The Xxxxxx Xxxx are designed as an independent living unit for functionally independent person who is capable of managing and directing his/her own life and possesses self-care abilities. Any provision of home care services to the tenant at the independent living unit is subject to the availability of resources, appropriateness of third party service providers and other circumstances, and HKHS assumes no responsibility to monitor, supervise, assess or provide guidance in relation to the tenant’s health care needs. As and when the tenant begins to encounter difficulty in activities of daily living, the tenant shall make or find suitable health care arrangements to be provided to the tenant in the premises, or consider transferring to a Care Facility capable of providing the required level of care to the tenant.

Independent Living. Additional meal services beyond the equivalent of one meal per day, additional linen, personal laundry service (except in Xxxxxx and Xxxxxxx as noted above), personal care and assistance, tray service beyond six trays per month.
Independent Living. The Trinity Pines, Casa Del Rio and Redwood Creek programs each offer independent living opportunities for clients referred by their guardian and/or county case management. The independent living housing is located on the same campus as the licensed residential service, affording the resident the optimal opportunity to successfully transition back into their community at the lowest level of care possible. Program Goals and Objectives Length of stay at Willow Xxxx varies in accordance with resident-specific needs. The Center has multiple programs that are designed to respond to both the short and long-term needs of residents in placement. Short-term care for residents is principally focused on personal wellness and recovery, and active discharge planning. Long-term care for some residents with an active mental health condition averages over twelve months. Residents may stay beyond this average length of stay depending on their mental health status. Treatment progress is reviewed at least monthly, or more often as necessary, by the treatment team, the resident’s guardian and county case management to determine ongoing service necessity. When appropriate the treatment team may recommend and coordinate with the guardian and county mental health agency for the resident’s transfer from one program to another at the Center in order to preserve placement in the least restrictive level of care or to facilitate transition to the lowest level of care possible. Independent Living. This does not mean living on your own or having to do everything yourself. All disabled people should have greater choice and control over the support they need to go about their daily lives; greater access to housing, education, employment, leisure and transport opportunities and to participation in family and community life.
Independent Living. People with disabilities, long term conditions or who become frail are able to live as safely and independently as possible in the community, and have control over their care and support.
Independent Living. Percentage of consumers enrolled in CMHC Community Support Services (CSS) that live in a private residence. This includes a consumer living with a spouse, friends, or family and who is capable of self-care. This category includes the consumer who is living independently with CSS support or CMHC financial support. The consumer in independent living is largely independent yet may choose to live with others for reasons not related to mental illness. Residing in this housing is not contingent upon participation in a specific treatment program.
