The Respondent agrees to voluntarily surrender his license to practice social work in the State of Iowa and understands that pursuant to 645 Iowa Administrative Code Section 12.1(5) a voluntary surrender, when accepted by the Board, shall have the same force and effect as an order of revocation. This agreement shall constitute Respondent’s written statement of intention to surrender his license pursuant to 12.1(5).

IT IS THEREFORE ORDERED. 11. Respondent’s license is hereby SUSPENDED for a period of twenty (20) days, effective August 12, 2013, to and including August 31, 2013. During the entire period of suspension, the Respondent shall not utilize any designation, abbreviation, or title which implies that Respondent is licensed to practice social work or otherwise hold herself out to be a licensed social worker. During the entire period of suspension, Respondent shall not engage in any activity or practice of social work in the state of Iowa for which a license is required.
IT IS THEREFORE ORDERED. 13. Upon completion of all administrative requirements, Applicant shall be issued a psychologist license, which shall be immediately placed on PROBATION for a period of two (2) years, subject to the following terms and conditions:
IT IS THEREFORE ORDERED. 12. Respondent shall be assessed a civil penalty in the amount of $500. This civil penalty shall be paid within thirty (30) days after the Board’s approval of this Settlement Agreement and Final Order, and shall be made payable to the Treasurer of Iowa and mailed to the Board. All civil penalty payments shall be deposited into the State of Iowa general fund.
IT IS THEREFORE ORDERED. 6. Upon approval of this Agreement by the Board, Applicant shall be granted a massage therapy license subject to a period of probation to end upon completion of compliance with the terms of this agreement. The probationary period shall commence on the execution date of this Agreement. The execution date is that date which accompanies the Board Chairperson or designee’s signature. During the probationary period, applicant shall comply with the following terms:
IT IS THEREFORE ORDERED. (1) That the Xxxxxx-Jal Unit Agreement is hereby approved.
IT IS THEREFORE ORDERED. 13. Respondent agrees to VOLUNTARILY SURRENDER his mental health counselor license to resolve this matter. This voluntary surrender is considered discipline and, when accepted by the Board, has the same force and effect as an order of revocation under 645 IAC 12.1(5).
