Licensure/Certification Status. The Operator does not have any arrangements with outside home care or personal care providers to serve residents. Should this change in the future, you will be provided with a listing of such providers with a statement describing the licensure or certification status of each provider and this will be updated as frequently as necessary.
Licensure/Certification Status. Per regulation at Title 10 of New York Codes, Rules and Regulations at Section 1001.8(f)(4)(iv), a listing of all providers offering home care or personal care services under an arrangement with the Operator, and a description of the licensure or certification status of each provider is set forth in Exhibit I.D. of this Agreement. Such Exhibit will be updated as frequently as necessary.
