Local Area Co-ordination Sample Clauses

Local Area Co-ordination. In Angus we are developing a model of local area co-ordination that takes forward the tasks but currently does not create a separate role. The tasks involved in local area co-ordination are described in “The Same as You” Person centred approaches to personal life planning is being introduced across all learning disability services in Angus. We aim to support individuals to have ordinary meaningful lives. This approach is being developed with our more traditional services as well as being core to the practice of newly developing services such as employment and future needs. Five staff act as consultant person centred planning consultatnts and a further 27 staff have been trained as personal centred planning facilitators. We have specifically appointed 2 Future Needs Co- ordinators to begin the personal life planning from the future needs assessment stage. The consultant facilitators have also provided management awareness sessions, facilitated consultation and organised training for families and self advocates. We aim to train all staff in personal life planning. The member of staff who works with the service user and their family and carers to develop a personal life plan may be part of a resource centre team, care management team, nursing team or care xxxx xxxxx depending on who is most appropriate for a particular person. The facilitators are currently supporting 25 people in the development of their personal life plans. Through planning these individuals have begun to lead ordinary lives over which they have control. These include people who have: • Moved to a house of their own from residential care; • Organised their own week undertaking activities of their choice and not attending day care; • Moved from the family home to a flat of their own; • Individualised support within the community; • Have paid employment; • Have reconnected with their family after years living in institutional and residential care. To specifically address an individuals community care needs, a single shared assessment process is being developed for use by joint learning disability community services which includes care management and learning disability nurses. The care plan derived from the single shared assessment forms part of the personal life plan for the individual. In taking forward this approach we have recognised that there is significant work to be undertaken to promote the inclusive communities agenda in the areas of employment and recreation.
