Loss and theft. 1. The Contractor must report the loss or theft of an OV-bike and/or the associated key as soon as reasonably possible to NS OV-fiets via the NS Customer Service (+ 31(0)00-000 00 00, local rate) or by email (xxxx@xx-xxxxx.xx). 2. While renting the OV-bike, the Contractor is liable for the loss or damage arising as a result of loss or theft of the OV-bike, up to a maximum amount of the then prevailing purchase value of the OV-bike as specified in the Rates List. 3. The Contractor is only liable for the excess (according to the Rates List), provided that the Contractor: - had locked the OV-bike using both locks provided; and - has reported the theft to the police an has send a copy of the police report to NS OV-fiets, and - in case of an OV-bike with a regular lock, has sent the key for the OV-bike to NS OV-fiets. The Contractor will send these evidence with an accompanying letter to the postal address specified at xxx.xx.xx/xx-xxxxx. 4. In the event that the Contractor reports an OV-bike rented by him as missing or stolen three (3) times within a period of eighteen (18) months, NS OV-fiets may decide to end the Subscription with immediate effect. 5. When the Contractor loses the key to the OV-bike, the Contractor must pay the replacement cost of the key according to the Rates List. The Contractor must return the OV-bike to the rental location, or will contact the NS Customer Service. In case that NS have to pick up the OV-bike, these transport costs will be be charged to the Contractor according to the Rates List, as well as the replacement costs of the lock. 6. In case of loss or theft of the OV-chipkaart while renting an OV-bike with an OV-fietsslot, the Contractor is obliged to return the OV-bike to the rental location. In the aforementioned case, the Contractor will return the OV-bike to the rental location or will inform the NS Customer Service so that the OV-bike can be picked up. In that case, the Contractor will be charged the amount stated in the Rates List for collecting the OV-bike. If the situation arises that between the moment of loss of the OV-chipkaart and the stolen / missing report an unlawful rental of an OV-bike has taken place, the same conditions apply that are applied to a stolen OV-bike. In order to claim the low deductible (Rates List), the Contractor must submit a copy of the official police report of the theft to NS OV-fiets.

In case of suspected theft by the worker, the Client must always notify the Contractor and immediately make a statement to the police and ask a copy of it. Only the worker can be prosecuted for theft.

Loss and theftThe Employer shall replace an Employees’ tools lost by burglary whilst working, provided that the tools were: (i) securely locked in an employer vehicle or locked private vehicle situated on the worksite; or, (ii) in any locked storage facility situated on the Employer’s premises, worksite, workshop or lockup area; or, (iii) in a securely locked employer vehicle, being used as directed by the Employer, parked overnight on private premises. A tool list with reasonable evidence, (may include photos and serial numbers) must be provided by the Employee and agreed to by the employer. Provision and maintenance of the employee’s tool list is the responsibility of the Employee and must be on file held by the Employer at the time of loss to enable an Employee to make a claim under this clause. Tools lost or stolen in accordance with this clause and not part of an agreed list between the Employer and Employee will be replaced up to a maximum value of $1300. This amount will be increased in accordance with the Award. Employee’s tools lost by theft not stored in accordance with this clause are the sole responsibility of the Employee and will not be replaced by the Employer.

Report of Loss (stop payment) Procedures (I) Bank card report of loss: The Applicant needs to complete the report of loss in writing at the counter, the personal service of the Bank’s telephone banking, Internet banking, or mobile banking, and other automated service systems that require password input. (II) Report of loss of other certifications agreed for transactions: The Applicant may complete the loss reporting procedures through the customer service of telephone banking, Internet banking, or mobile banking, and may go through the reissuance or change procedures in person at the counter during business hours. The Bank may suspend all the over- the-counter transaction services to the account holder before the card reissuance or change procedures are completed. III. If all the transactions conducted by the Applicant before reporting the card loss to the Bank or before completing the card reissuance or change procedures, in the event that the specimen signature, bank card, and other certifications agreed for transactions are genuine, then the payment by the Bank to the Applicant is with the effects of performance. IV. The Applicant agrees that the transaction related certificate photocopies, certificate pictures, audio recordings, or computer-stored information retained by the Bank have the same validity as the original certificates and can be used as proof of all the transactions conducted by the Applicant.

Loss and theft. The Card Holder is obliged to sign the back of the Card immediately upon receipt, and to keep the Card in a safe place. The personal PIN code shall be kept separate from the Card and not disclosed to unau- thorised third parties. If the Card is lost or stolen, the card issuer must be informed immediately. All damages resulting from theft, loss or misuse of the Card shall be borne by the Card Holder. The Card Holder is liable for all costs incurred by the card issuer related to such a theft of loss of the Card. A fee of CHF 10.00 may be charged to replace a lost, stolen or damaged Card.
