Maintenance and Provision of Certain Information. Dentist acknowledges that MA Plan is required under applicable federal law and regulations to submit to CMS certain information regarding the benefits provided by MA Plan and quality and performance indicators, including but not limited to, (i) disenrollment rates of MA Members enrolled in MA Plan for the previous two years; (ii) information regarding MA Member satisfaction with MA Plan and (iii) information on dental outcomes. Dentist acknowledges that MA Plan is also required under such laws and regulations to disclose certain information to MA Members in such form and manner requested by CMS. Dentist shall maintain all records and reports reasonably requested by CMS and/or MA Plan and shall provide such records and reports to CMS and/or MA Plan as reasonably requested, to enable MA Plan to meet its obligation to submit such information to CMS and to disclose certain information to MA Members as required by applicable law and regulations.
