Common use of Medical Care Clause in Contracts

Medical Care. Undersigned authorize the Released Parties to call for medical care for Participant or to transport Participant to a medical facility or hospital if, in their opinion, medical attention is needed. Undersigned agree to pay all costs associated with such medical care and related transportation.

Medical Care. Undersigned authorize the Released Parties to call for medical care for Participant Holder or to transport Participant Holder to a medical facility or hospital if, in their opinion, medical attention is needed. Undersigned agree to pay all costs associated with such medical care and related transportation.

Medical Care. Undersigned authorize the Released Release Parties and/or their authorized personnel to call for medical care for Participant or to transport Participant to a medical facility or hospital if, in their opinionthe opinion of such personnel, medical attention is needed. Undersigned agree agrees to pay all costs associated with such medical care and related transportation. I am aware and understand that I should carry my own health insurance.
