Medical Transportation. HHSC reserves the right to amend the scope of the Contract to include medical transportation services (MTP) for Medicaid Members. For additional information regarding the MTP Program, the MCO should refer to the Nonemergency Medical Transportation (NEMT) Full Risk Broker Services RFP. MCOs should note that the MTP Program includes numerous Xxxx x. Xxxxx requirements, including enhanced call center performance standards. If MTP services are added to the scope of the Contract, HHSC will provide advance written notice and conduct appropriate Readiness Review.

Medical Transportation. CMHCC, through its University Providers, shall provide or arrange for EMS transportation of patients to the UTMB/TDCJ Hospital in Galveston and / or freeworld hospitals based on an assessment and clinical evaluation by a qualified health care professional. TDCJ will be responsible for transportation by chain bus or other non-EMS transportation services. 1. For transfer of inpatients between a TDCJ infirmary or regional medical facility and the UTMB/TDCJ Hospital at Galveston or a free world hospital, EMS transportation shall be routinely provided. 2. TDCJ transportation may be utilized for inpatient transfers provided that: a. offenders transferring from one inpatient facility to another inpatient facility shall be assessed by the sending physician and/or mid-level practitioner; b. documentation charted or noted that the offender is not anticipated to require medical intervention or assessment while enroute; and c. the transport time by TDCJ van is less than three hours. 3. If the criteria for TDCJ transport in paragraph 2 (a) and (b) is met, but the transport time is greater than three hours, the request for TDCJ transport shall be referred to the TDCJ Health Services Liaison for review and decision.

Medical Transportation. 23.01 Employees who have qualified under Article 21 shall be eligible for reimbursement of medical transportation costs or emergency dental treatment transportation costs as provided in this Article. Employees who work in Xxxxxx Xxxxx, Inuvik or Tuktoyaktuk will be eligible for benefits under this Article. 23.02 Where a medical or dental practitioner certifies that it is necessary for an eligible employee, the employee's spouse, or the employee's dependent child who is solely supported financially by the employee, to receive medical treatment or emergency dental treatment not available at the work location, reimbursement for return air transportation or the equivalent from work location to the nearest centre where treatment can be provided and/or reimbursement of ambulance costs shall be provided by the Company. The Company mileage rate shall apply where air transportation is not available. 23.03 Where a qualified medical or dental practitioner certifies that it is necessary, under conditions of Clause 23.02, for an employee, the employee's spouse, or the employee's dependent child who is solely supported financially by the employee to be accompanied by another person, reimbursement for return air transportation costs shall be provided for that person by the Company. Such shall not apply if the employee is driving. 23.04 Should medical or emergency dental treatment not be available at centres closer than Edmonton, return transportation to Edmonton shall be provided. In no event will reimbursement costs exceed the equivalent return air transportation to Edmonton. 23.05 Medical transportation and emergency dental treatment transportation shall not be provided if such is provided through other Government or other employer plans. In the event that partial reimbursement is available through other plans, the Company shall provide reimbursement for the difference. 23.06 Should an employee's spouse be employed by another employer who provides medical or emergency dental transportation and the spouse is eligible for such assistance, the spouse shall not be eligible for medical or dental transportation under this Article. 23.07 Medical or emergency dental transportation for dependent children which is also provided by the employer of an employee's spouse shall only be paid by the Company where the employee is the principle income earner of the family. 23.08 For the purposes of the Article, any medical certification, receipts, documentation, or communication from oth...

Medical Transportation. Where XXXX’s medical staff, in conjunction with a Member’s attending physician, determine medical transportation to the nearest appropriate medical facility, or to Canada, for treatment is necessary, AZGA will arrange all aspects of the transport, including ground transport to and from the hospital and airport, at the point of departure and arrival. The medical staff of AZGA will also arrange for medical accompaniment, if necessary.
Medical Transportation. If medically necessary, arrangements will be made to transfer an insured person to and from the nearest medical facility or to a medical facility in the insured person’s province of residence. Expenses incurred for the medical transportation will be paid, as described under Medical Services and Supplies Ambulance. Medical Transportation. 26.01 Emergency travel, for all employees, spouses or dependants who may require medivac on a scheduled airline or emergency charter will be paid for by the Company in the event funding is not available from other sources. 26.02 The Company has a Medical Travel Policy available to employees and immediate family dependents living in the NT to access non-elective medical treatment that is not available locally. Details of the medical policy are available from the local Human Resources Advisor. 26.03 Changes to the Medical Travel Policy will be reviewed with the Union to explain reasons for changes prior to implementation. 26.04 In cases where the patient is a child requiring accompaniment, the Company will pay the airfare for both the child and an accompanying parent or guardian. 26.05 For maternity delivery trips, the Company will pay for the travel of the employee, spouse and children to the center chosen for delivery, up to the equivalent cost of economy air travel to the employee’s point of hire. The travel costs of the husband’s and children’s maternity trip will be taxable benefits to the employee; any tax arising from this taxable benefit will be the employee’s responsibility. 26.06 Employees or dependents will be reimbursed for out of pocket expenses, supported by receipts, up to the amount of $200.00 per day plus $15 per day for each additional person traveling as a patient or escort anywhere in the Northwest Territories for a maximum period of four days. The employee will be reimbursed for such things as meals and hotels where they receive outpatient care or are admitted to a medical or dental center. For treatment etc. in the south, the reimbursable amount is $150.00 per day plus $15 per day for each additional person travelling as a patient or escort for a maximum period of four days. If the patient is able to travel in a period less than four days, then it is expected the flights will be booked and travel costs will be charged accordingly.
