NOTIFICATION OF DEATH. 23 A. Upon becoming aware of the death of any person served pursuant to this Agreement, 24 CONTRACTOR shall immediately notify ADMINISTRATOR.
25 B. All Notifications of Death provided to ADMINISTRATOR by CONTRACTOR shall contain 26 the name of the deceased, the date and time of death, the nature and circumstances of the death, and the 27 name(s) of CONTRACTOR’s officers or employees with knowledge of the incident.

NOTIFICATION OF DEATH. Upon becoming aware of the death of any person served pursuant to this Agreement, 29 CONTRACTOR shall immediately notify ADMINISTRATOR.
NOTIFICATION OF DEATH. 21.1 The policyholder, beneficiary and/or surviving relative is obliged to notify the insurer of the death of the insured party as soon as possible, but at the latest within 4 months after the death occurred. If this is not complied with, the insurer shall be entitled to withhold payment.
21.2 The insurer shall not be released from its obligation to deal with a report if it can be demonstrated that due to force majeure the report could not be made within the set period and that the insurer's interests were not harmed by the late report.

CONTRACTOR shall notify ADMINISTRATOR by telephone immediately upon 33 becoming aware of the death due to non-terminal illness of any person served hereunder or served 34 within the previous twelve (12) months; provided, however, weekends and holidays shall not be 35 included for purposes of computing the time within which to give telephone notice and, notwithstanding 36 the time limit herein specified, notice need only be given during normal business hours.

8 2. In addition, CONTRACTOR shall, within sixteen (16) hours after such death, hand deliver 9 or fax, a written Notification of Non-Terminal Illness Death to ADMINISTRATOR.
10 3. The telephone report and written Notification of Non-Terminal Illness Death shall contain 11 the name of the deceased, the date and time of death, the nature and circumstances of the death, and the 12 name(s) of CONTRACTOR’s officers or employees with knowledge of the incident.
13 B. TERMINAL ILLNESS DEATH
14 1. CONTRACTOR shall notify ADMINISTRATOR by written report faxed, hand delivered, 15 or postmarked within forty-eight (48) hours of becoming aware of the death due to terminal illness of 16 any person served hereunder. The Notification of Terminal Illness Death shall contain the name of the 17 deceased, the date and time of death, the nature and circumstances of the death, and the name(s) of 18 CONTRACTOR’s officers or employees with knowledge of the incident.
19 2. If there are any questions regarding the cause of death of any person served hereunder who 20 was diagnosed with a terminal illness, or if there are any unusual circumstances related to the death, 21 CONTRACTOR shall immediately notify ADMINISTRATOR in accordance with 22 subparagraphSubparagraph A. above. 23

To register the death we will need to see the original UK Grant of Representation, or a sealed office copy (we are not able to accept certified copies). This could be: Grant of Probate; Letters of Administration; or Certificate of Confirmation (Scotland). If the relevant shares are held on behalf of more than one person, and after the event the shares are to be held on behalf of the other person(s) then we will arrange for the shares to be transferred into their name(s) to remain in Share service. In order to complete the transfer of shares into new name(s) after the event, we may need to request additional information and until this information is available the shares will continue to be held in the original name(s).

9 a. NON-TERMINAL ILLNESS CONTRACTOR shall hand deliver, fax, and/or send 10 via encrypted E-Mail to ADMINISTRATOR a written report within sixteen (16) hours after becoming 11 aware of the death due to non-terminal illness of any person served pursuant to this Contract.
12 b. TERMINAL ILLNESS CONTRACTOR shall notify ADMINISTRATOR by written 13 report hand delivered, faxed, sent via encrypted E-Mail, within forty-eight (48) hours of becoming 14 aware of the death due to terminal illness of any person served pursuant to this Contract.
15 c. When notification via encrypted E-Mail is not possible or practical CONTRACTOR 16 may hand deliver or fax to a known number said notification.
17 C. If there are any questions regarding the cause of death of any person served pursuant to this 18 Contract who was diagnosed with a terminal illness, or if there are any unusual circumstances related to 19 the death, CONTRACTOR shall immediately notify ADMINISTRATOR in accordance with this 20 Notification of Death Paragraph. 21
