ORGANIZATIONAL NAME CHANGE. The Contractor shall submit written notification to the Department of any changes in the Contractor’s name, address, telephone number, facsimile number and/or e-mail address with an effective date of such change. The Contractor shall submit to the Department a copy of any registration “to do business as,” “DBA,” or “also known as,” “AKA,” and any legal corporate name change filed with the Secretary of State.
