Personnel records may include without limitations, the following: A. Application for employment. B. Copy of latest contract, properly signed. C. Copy of latest salary notice. D. Medical examination form. E. Current Ohio teaching certificate for subject area. F. Teacher’s Experience Record. G. Personal and professional data form.
