PROJECT EVALUATION/REPORTING REQUIREMENTS. Using Agency provided reporting templates, Grantee will submit required reports, related reports and information as Agency may reasonably require. Required reports include Quarterly Reports and the Final Report. Grantee must submit the reports as indicated below: REPORT DUE DATE Quarterly Reports Within 30 days after the end of each quarter listed below : Quarter 1: July 1, 2021 – September 30, 2021 Quarter 2: October 1, 2021- December 31, 2021 Quarter 3: January 1, 2022 – March 31, 2022 Quarter 4: April 1, 2022 - June 30, 2022 Quarter 5: July 1, 2022 – September 30, 2022 Quarter 6: October 1, 2022 - December 31, 2022 Quarter 7: January 1, 2023 – March 31, 2023 Quarter 8: April 1, 2023 - June 30, 2023 Final Report By August 1, 2023 If the Grant is terminated in accordance with Section 18, the final report is due within 30 days after the termination date. Final payment is contingent upon Agency’s acceptance of the Final report. The obligation to deliver the Final Report shall survive the termination of this Agreement. Agency will evaluate Grantee as reimbursement requests are made by the Grantee using the reimbursement form provided by the Agency. Agency will use data provided in reimbursement requests to monitor the Grantee. Grantee’s failure to use the required reimbursement form may result in a delay or denial of reimbursement. If the Performance Period begins prior to the Executed Date, any reports for Project activities shown in this Exhibit A as due prior to the Executed Date must be provided to Agency within 30 days of the Executed Date, if not already provided to Agency despite the lack of an executed Grant. Grantee will not be in default for failure to perform any reporting requirements prior to the Executed Date.

PROJECT EVALUATION/REPORTING REQUIREMENTS. Grantee will be required to submit quarterly reports that detail program progress and performance for reimbursement. Quarterly progress reports will be due within 30 days of the end of each quarter (or on the date designated by Agency for report submission). The grantee will address all clarifying questions and make any necessary corrections promptly. Reports must be received and approved for grant reimbursement claims to be processed. Reports must include: • Quarterly narrative report indicating description of activities, challenges, progress on outcomes, and promising practices during quarters; and • Quarterly data outputs and outcomes report; and • Quarterly expenditure report detailing all expenses associated with the grant using funds for allowable expenses and services and maintaining required financial records; and funds subcontracted to partners, as well as direct payments to youth (such as incentives and other direct individual support services). The Program Area and project selected for funding must, at a minimum, designate a qualified staff person, to ensure that federal performance measures data are collected and reported. The Grantee must collect performance measures data required by the OJJDP for Title II Formula Grant Program Areas in the specific program area of “Title II Formula Grants Program Performance Measures” output numbers 1, 14, 15, 17, 21, 24, 27, and 28 found at: xxxxx://xxxxx.xxx.xxx/funding/grant-performance-measurement/performance-measures If the Performance Period begins prior to the Executed Date, any reports for Project activities shown in this Exhibit A as due prior to the Executed Date must be provided to Agency within 30 days of the Executed Date, if not already provided to Agency despite the lack of an executed Xxxxx. PROJECT EVALUATION/REPORTING REQUIREMENTS. Agency will provide technical assistance to Grantee to develop the Work Plan and will periodically conduct site monitoring visits. Site monitoring visits include, but are not limited to, Agency Grant Manager: • meetings with the Grantee; • observing reengagement activities and sites; • verifying Grantee data collection; • reviewing expenditure reports; • reviewing case management practices and case notes; and, • reviewing other processes relevant to the Grant and reporting requirements. Grantee will submit the following reports to the Agency Grant Manager identified in SECTION 4 and Work Plan on a quarterly basis, in the format prescribed by the Agency Grant Manager: • Narrative: description of program activities, including successes and opportunities for improvement, innovative practices, and participant/case management stories during the respective quarter; • Key Performance Measures (KPM): captures data about the Grantee’s reengagement program outreach and overall program enrollment/participation;

Upon completion (on or before September 30, 2024), Grantees will submit to Agency a comprehensive report to include but not be limited to: Student assessment tools and student outcome data Data showing fidelity of implementation of culturally affirming practices, including cultural identity development, leading towards improvement in student outcomes Data showing the aggregated impact of response to intervention processes regarding unfinished learning through academic and mental health supports Description of leadership and self-advocacy skills implemented A recommendation for future statewide systems of support to increase culturally affirming practices for all students Grantee must provide to the Agency additional data as requested.
