Removable Appliance Therapy. An appliance that is removable and not cemented or bonded to the teeth • Fixed Appliance Therapy – A component that is cemented or bonded to the teeth • Other Complex Surgical Procedures – Surgical exposure of impacted or unerupted tooth for orthodontic reasons; or surgical repositioning ofteeth • Repair or replacement of brokenappliances; Treatment that is already in progress with appliances placed before You were covered by this Agreement will be covered on a pro-rated basis. Orthodontic Payments Because orthodontic treatment normally occurs over a long period of time, benefit payments are made over the course of treatment. You must continue to be eligible under the Agreement in order to receive ongoing payments for Your orthodontic treatment. Payments for treatment are made:
