Reporting a Lost or Stolen Card. If your card is lost or stolen report immediately to either the Card Office 000-000-0000, CCSU Police Department at 000-000-0000, or log on to blue- xxxx.xxxx.xxx. Once notification of a lost or stolen card is made, your Blue Chip Account will be put on "hold," pre- venting further transactions on the account. Upon issuance of a replacement Blue Chip Card, your re- maining cash balance will automatically be trans- ferred to your new Blue Chip Card. Please refer to the “Liability” section on the back of this brochure for unauthorized use of the card. Card Replacement Fees First card replacement: $10; subsequent card re- placements $25 each. No replacement fees for dam- aged cards unless damage is a result of card misuse. Refund of Your Blue Chip Account Balance Your Blue Chip Account is designed to be a continu- ous benefit and service to you for your entire career as a student, faculty, or staff member at Central Con- necticut State University (CCSU). You cannot withdraw cash from your Blue Chip Account. You should anticipate spending your account balance or leaving the balance intact over the holidays and school breaks. Your account will remain from semes- ter to semester, year to year and will not be refunded unless the cardholder withdraws, graduates or is dis- missed from CCSU or for employees whose employ- ment with CCSU is terminated. Upon separation from CCSU, your account will be reviewed and you may be eligible for a refund of any balance remaining after all University debts have been paid on your Bursar’s Office account. In such cases your refund will be sent via direct deposit to your last known bank- ing information that had been provided to the Univer- sity. If no known bank account information is available or has become invalid a check will be mailed to your last registered University address. If your refund will be less than $5.00, the refund will be dispersed at the dis- cretion of the University. All non-student/non-employee cardholder accounts that have been inactive for 12 months will also be re- viewed and may be eligible for a refund in the same manner as outlined above.

Reporting a Lost or Stolen Card. You are responsible for the possession and use of the Card and must notify us immediately in the event of loss, theft, or possible unauthorized use of the Card by calling us at 0-000-000-0000 or 000-000-0000 in the Reading, PA area, between 8:00 a.m. and 5:00 p.m. on any Monday – Friday business day (8:00 a.m. to 3:30 p.m. on Wednesday) or e-mail us during these times at Xxxxxxxxxx@xxxx.xxx. After hours, you must telephone us to report a lost or stolen card.

  • Lost or Stolen Card Except as otherwise stated herein, you are responsible for all transactions initiated through the use of your Card, including any PIN-based transactions, any transactions initiated by presenting the Card Number only and any authorized transactions initiated by someone else. If you believe your Card or Access Information has been lost or stolen, call the number or write to the address listed in the “Contact Information” section below. You should also call the number or write to the address listed in the “Contact Information” section below if you believe a transaction has been made using the information from your Card or Access Information without your permission. You also agree to cooperate completely with the Servicer in attempts to recover funds from unauthorized users and to assist in their prosecution.

  • Lost or Stolen Cards You must tell us immediately by telephone or in writing if you know or suspect that a Card is lost or stolen, or if you know or suspect that your Card or the Account is being used without your authority. Our toll free number is 0-000-000-0000, or collect at (000) 000-0000. Once you have told us that your Card or Account is lost, stolen or used without your authority, we will be able to prevent use of that Card and the Account number.

  • LOST, STOLEN OR DAMAGED CARDS If the Cardmember loses the Card or if the Card is damaged or stolen or if the PIN became available to any other persons, the Cardmember must immediately inform AEME by telephone. In addition, the Cardmember shall provide AEME with a written notification within 3 days from the date of the occurrence of any of the above listed events, as per applicable laws. All notices pursuant to this article must be sent to the following address: AMEX (Middle East) B.S.C. (c), P.O. Box 5990, Manama, Kingdom of Bahrain. Or facsimile number: (+000) 00 000000. To inform AEME by phone, call the number indicated in the Cardmember Additional Information Leaflet. If AEME is duly informed in the manner described above the Cardmember shall not be liable for any Charges made on his or her Card by any third parties after the Card has been reported lost or stolen. Failure to comply with the provisions of this article will render the Cardmember liable for all Transactions made on his or her lost or stolen Card until the time the Card is reported as lost or stolen to AEME. However, the Cardmember shall at all times be fully liable for all Charges including Cash Withdrawals even if these Charges were made by third parties if these third parties gained possession of the Card or any Supplementary Card or the PIN with the knowledge, permission, approval or other acts of the Cardmember or any Supplementary Cardmember or because of their negligence. If the Card is found after it was reported lost or stolen, the Cardmember may not use or attempt to use the Card at issue and shall immediately inform AEME which will take the appropriate action.

  • Mutilated, Destroyed, Lost or Stolen Certificates If (a) any mutilated Certificate shall be surrendered to the Certificate Registrar, or if the Certificate Registrar shall receive evidence to its satisfaction of the destruction, loss or theft of any Certificate and (b) there shall be delivered to the Certificate Registrar and the Owner Trustee, such security or indemnity as may be required by them to save each of them harmless, then in the absence of notice that such Certificate shall have been acquired by a bona fide purchaser, the Owner Trustee on behalf of the Trust shall execute and the Owner Trustee, or the Certificate Registrar, as the Owner Trustee’s authenticating agent, shall authenticate and deliver, in exchange for or in lieu of any such mutilated, destroyed, lost or stolen Certificate, a new Certificate of like tenor and denomination. In connection with the issuance of any new Certificate under this Section, the Owner Trustee or the Certificate Registrar may require the payment of a sum sufficient to cover any tax or other governmental charge that may be imposed in connection therewith. Any duplicate Certificate issued pursuant to this Section shall constitute conclusive evidence of an ownership interest in the Trust, as if originally issued, whether or not the lost, stolen or destroyed Certificate shall be found at any time.
