There are benefits and limitations to this service. Some of the benefits may include increased access to care, less travel, and convenience. You may benefit from telebehavioral health services, but results cannot be guaranteed or assured. You understand that there are risks and consequences of participating in telebehavioral health services, including, but not limited to, the possibility, despite best efforts to ensure high encryption and secure technology on the part of your therapist, that the transmission of your information could be disrupted or distorted by technical failures; the transmission of your information could be interrupted by unauthorized persons; and/or the electronic storage of your medical information could be accessed by unauthorized persons. Dealing with technological issues during sessions may result in added complications and emotional distress. In addition, you understand that telebehavioral health services and care may not be as complete as face-to-face services; missing information may make if more difficult for your therapist to understand your problems and/or help you get better. Because telebavioral health is a relatively new service and quickly evolving, there may be other risks not yet identified. You always have the right to request alternatives to telebehavioral health services. The provision of telebehavioral health services is limited in several ways. Telebehavioral health services is only offered to clients who reside in the state where the therapist is licensed to provide care, and the clients must be physically located in that state during the telebehavioral health services sessions. If you become located in a different state for any reason you must notify your therapist. You also understand that if your therapist believes telebehavioral health services is not appropriate for you at any point, you will be referred to a different professional for in person services; although your therapist would attempt to locate a different professional near you, if there are none, you may have to travel a considerable distance to access care. You also understand and accept that telebehavioral health services do not include provision of emergency services. If you are experiencing an emergency, you should call 911 on a separate phone line and request help.
Risks and limitations of antibody incompatible transplantation Despite established and successful desensitization protocols (with the avoidance of hyperacute rejection in an antibody incompatible setting) problems and limitations do exist. In a landmark study of 738 patients who underwent live-donor ABOi kidney transplantation compared to matched controls, Montgomery et al, report a higher incidence of graft loss in the ABOi group compared to the compatible group (27.1% at 10 years compared with 23.9%, p=0.001). They state this was largely due to graft loss during the first 14 days post- transplant (73). Figure 1.7 Graft loss comparing ABOi vs ABO Compatible Matched Controls Clearly, antibody incompatible transplants are at higher risk of early aggressive AMR (Figure 7) compared with compatible transplants, especially when associated with a rise in antibody titres in the early post-operative period increasing the risk of severe allograft injury (74). In addition, despite terminal complement inhibition with eculizumab showing significantly reduced early AMR in positive crossmatch renal recipients (75), Xxxxxxx et al report the incidence of early AMR in recipients despite C5 inhibition - possibly be due to complement-independent mediated pathways or the presence of high levels of IgM DSA in these reported patients that had AMR despite eculizumab treatment (76). They suggest that IgM may amplify the effects of C3 breakdown and hence inflammation and allograft injury via C3a (anaphylatoxin, chemotactic agent promoting cellular infiltration) and C3b (promotes leucocyte migration). In addition, in vitro studies have shown that both IgM and MBL can activate the alternative pathway which may also explain further cell injury independent of C1q/classical pathway activation (76) (77). Another major consideration in AIT is the fact that not all patients may be clinically suitable to undergo the physiologically intense desensitization protocols involving multiple sessions of plasma exchange, immunoadsorption and high dose immunotherapy and bleeding risk post operatively (78). Potential recipients must demonstrate a relative robustness to undergo such preoperative treatment – an essential pre-requisite, which may exclude more frail and elderly patients from having access to this modality of transplantation.
